What are the responsibilities and job description for the Registered Nurse (RN) position at Surprise Valley Health Care District?
SUMMARY:
Works in conjunction with physicians, other nursing staff, residents, families and other facility departments to provide optimum individualized nursing care. The Registered Nurse and Surprise Valley Health Care District is a professional nurse responsible for the delivery of quality patient care for the Acute patient and for the Skilled Nursing Facility patient. Their job description lists the areas in which the nurse will be held accountable.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Attends CPR classes and other in-service education classes related to job position. Pass Skills Competency Test. Participates in monthly staff meetings. ACLS certification required and PALS certification recommended.
- Will follow written dress code guidelines and at all times present a professional appearance.
- Will keep current on the personnel policies, procedure manuals, policy manuals and other manuals pertinent to nursing. (Fire, Disaster, Coroner, MSDS, Infection Control, Nursing, Pharmacy, etc.)
- Will read and understand the “Patient Rights” for Skilled Nursing Facility and for Acute patients.
- Will be knowledgeable of all new directives and/or changes pertaining to nursing.
- Will coordinate, supervise and guide the activities of the patient care and staff for their shift according to their role for that particular day. Is responsible as Charge Nurse for Acute, including ER, and SNF, or in another capacity as the patient load demands.
- Will be responsible to see that all records pertaining to their shift are completed and signed according to the policy of Surprise Valley Hospital.
- Provides direct patient care as needed, assures that call lights are answered promptly, and that nursing care assigned to assistants is completed and documented according to facility policy/procedure.
- Will be able to provide professional daily bedside care to acute-skilled nursing patients, perform all nursing treatments, use all equipment properly and supervise and instruct other staff members in procedures and treatments whenever needed.
- Will administer all IV therapy as ordered by physician. Will be certified as capable of instituting IV therapy according to Surprise Valley Hospital certification form.
- Will instigate all emergency treatments and protocol according to Surprise Valley Hospital policy manual. Responds to emergency situations appropriately, notifies all staff, family, and physician in a timely manner. Carries out emergency orders quickly and efficiently and documents same in the resident’s record. Does follow-up as needed.
- Will make rounds and see all patients in their assignment daily. Is responsible for all patient care in their particular role for that day including medication, treatments, bedside care, activities and problems that might arise.
- Makes rounds on each resident in their assignment daily, assesses all new residents and any resident who is ill, has fallen, or has sustained an injury and any who have had a change in condition. Documents assessment per facility policy/procedure and does appropriate follow-up as indicated.
- Will attend, participate, or institute in-service programs. Records of attendance or participation will be kept on file with DSD. Will attend yearly competency evaluation.
- Registered Nurse on duty may be required to assume Director of Nursing duties in the absence of the Director o Nursing.
- Follows facility policy/procedure when administering medication and documents same. Assures that medications are correct, not outdated, are in adequate supply and administered as ordered.
- Receives and gives report on resident’s condition, assigns nurses aides to care for individual residents and assures that they are aware of nursing care to be delivered.
- Notifies physician or FNP of any changes in the resident’s condition or current needs, documents and notes doctors’ orders per facility policy/procedure. Carries out all physician orders in a timely manner.
- Completes the assessment of new residents, in developing the plan of care and assuring that care is carried out in accordance with the care plan. Follows-up on any problems or concerns that the resident or family may have.
- Documents daily as per hospital policies.
- Performs nursing procedures according to facility policy/procedure while keeping safety and infection control principles and practices in mind at all times. Operates equipment in a sage and effective manner.
- Discharges residents/patients assuring that they are given complete discharge instructions according to facility policy/procedure.
- Assures that all residents/patients and co-workers are treated with dignity and respect. Assures residents/patients are allowed privacy as needed.
- Promotes good working relationships with other departments, co-workers, residents, families and physicians. Is supportive of the facility in the community. Maintains confidentiality and uses discretion in discussing hospital matters.
- Participates in department or unit quality improvement activities, compliance activities and HIPAA activities.
- Attends monthly nursing staff meetings.

SUPERVISORY RESPONSIBILITIES:
Directly supervises 2 to 6 employees in the Nursing Department. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
EDUCATION and/or EXPERIENCE:
Graduate of accredited school of nursing, one to two years related experience and/or training; or equivalent combination of education and experience is desired. Previous experience in acute and/or geriatrics preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must possess a current California Registered Nurse License, CPR certification, ACLS certification and PALS certification required.
