Position Summay:
Provides leadership to the nutrition team at the Community Health Team. Performs and oversees direct patient medical nutrition therapy activities. These include nutritional assessment in the outpatient setting, evaluation of laboratory values and other pertinent medical data, and participating in developing appropriate care plans for nutrition support. Monitors the nutritional status of patients and provides counseling for patients on special diets, consults with other health care professionals (physicians, nurses, pharmacists, etc.) as appropriate, participates in departmental quality improvement activities, policy and procedure development, and develops standards of care. Assists in the management of labor and resources at the Community Health Team, in order to provide evidence-based nutrition therapy; superb patient care and high patient satisfaction. In day to day responsibilities ensures compliance with regulatory standards and works toward improving systems and processes. Acts as DSME coordinator and provides diabetes self-management education to patients and staff in accordance with the American Diabetes Association Guidelines.
Qualifications:
- EDUCATION:
- Bachelor’s degree in Nutrition or Dietetics with completion of American Dietetic Association approved undergraduate program, dietetic internship or master’s degree with qualifying work experience.
- EXPERIENCE/SKILL SETS:
- Two to three years of outpatient nutrition services.
- Clinical Nutrition Management experience preferred.
- Must be able to sing Bohemian Rhapsody from start to finish
- LICENSURE/CERTIFICATION(S):
- Registered by the Commission on Diabetic Registration for the Academy of Nutrition and Dietetics
- Certified Diabetes Educator
