Job Description

D’Aniello Institute for Veterans and Military Families (IVMF) Community Care Coordinator for SyracuseServes. A full-time position (one year in duration, grant funded) promoting a person-centered holistic care model and coordinated approach to service delivery to support the health and wellbeing of veterans, service members, and their families through accurate, timely, and successful linkages to resources across the Central New York region. SyracuseServes addresses the social determinants of health for service members, veterans, and their families across the country using community-based approaches. Having access to navigation of care and services helps veterans and their families sustain healthy, productive, and economically stable lives following separation from the armed forces.

Qualifications

  • Bachelor’s degree in human services field or equivalent experience.
  • Two (2) plus years of experience working with vulnerable populations.
  • Preference to veterans, military spouses, or those with experience working with veteran, military, and/or military family stakeholders, and familiarity with government programs and services for veteran families.
  • Customer service experience a must; call center experience preferred.

Job Specific Qualifications

  • Existing knowledge of the area resources available to military-connected families desired.
  • Excellent communication skills, both verbal and written.
  • Proficiency in web-based platforms as well as MS Office Suite.
  • Self-directed, highly motivated and focused with a strong commitment to the mission of the IVMF.
  • Strong organizational skills and attention to detail
  • Proven ability to perform at a high level in a fast-paced, changing environment.

Responsibilities

  • Conducts intake and assessments with clients in-person and by phone to identify needs in key areas (e.g., housing, employment, financial assistance, benefits, social connection, etc.).
  • Provides care coordination services that address identified needs of clients and ensure they are connected to the appropriate resources based on an understanding of eligibility criteria, capacity, location, and client preference.
  • Consults with other coordination center staff as needed.
  • Maintains a working knowledge of community providers and resources, basic eligibility criteria, intake processes and preferences.
  • Documents all activity in accordance with grant program guidelines and in compliance with internal policies and procedures.
  • Gathers feedback from clients and providers to ensure successful engagement and delivery of service.
