What are the responsibilities and job description for the SIL Case Manager (Hybrid) position at Tabor Children's Services?
The supervised independent living (SIL) case manager provides support and directions for youth and develops an individualized life skills program for each youth. The SIL worker demonstrates and teaches the necessary skills for youth to live independently. The SIL worker locates appropriate community resources, establishes links, and advocates on behalf of the youth. They will implement service plans relative to emancipation and coordinate ancillary services with the youth. Additionally, the SIL worker acts as a resource to Tabor staff regarding adolescent and independent living issues. Hours are from 9 a.m. to 5 p.m. Monday through Friday, but may vary according to organizational needs. Regular travel is required.
EDUCATION
Bachelor’s degree in Social Work or related field.
EXPERIENCE
Two (2) years of experience working within child, youth and family service systems.
