POSITION SUMMARY
Coordinates the provision of medically-related social services to residents, allowing residents to attain the highest physical, mental, and psychosocial well-being.
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Obtain and document the resident's psychosocial history upon admission
- Quarterly (and as needed) evaluation of resident psychosocial status, review of plan of care identifying and seeking ways to support residents' individual needs and preferences, customary routines, concerns and choices.
- Serves as a member of the Interdisciplinary Team (IDT)
- Communicate with IDT, resident and responsible party to ensure that resident's individual needs are met
- Provide discharge planning services by working with IDT, resident, resident family and physician to ensure a safe transition to alternate setting.
ESSENTIAL SKILLS
EDUCATION and / or EXPERIENCE
A minimum of 1 year working in geriatrics.
