We are now taking the next step in our service history by providing Addiction Treatment with a special emphasis on opiate addiction and mental health. Talbot Health Services MAT (medically assisted treatment) program combines prescription medications, counseling and behavioral therapies to address our clients’ substance abuse disorder (SUD) needs. The services we offer take a whole person approach to treating SUD. As a member of our Mansfield, Ohio, team you will be part of an Intensive Outpatient Treatment Program individualized to address a client’s needs.
SUMMARY:
The Case Manager (CM) provides case management services, as defined by defined by Ohio Department of Mental Health and addiction Services (OhioMHAS). CM staff provides an array of services delivered by community based, mobile individuals or multidisciplinary teams of professionals and trained others. Services address the individualized mental health needs of the client. They are directed towards adults and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual. The purpose/intent of CM services is to provide specific, measurable, and individualized services to each person served. CM services should be focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in work, family and integration and contributions within the community. They may also provide or assist with transportation. A high level of professionalism and flexibility is essential for the individual to complete job responsibilities.
Working under the supervision of the Clinical Director, s/he is responsible for designing and implementing client services that address the TCSO mission and are provided in a manner consistent with the organization’s vision and values. This is a salaried position with hours and schedule established to complete required duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Ongoing assessment of client needs;
2. Assistance in achieving personal independence in managing basic needs as identified by the individual;
3. Facilitation of further development of daily living skills, if identified by the individual
4. Coordination of the ISP, including:
A. Services identified in the ISP;
B. Assistance with accessing natural support systems in the community; and
C. Linkages to formal community service/systems;
5. Symptom monitoring;
6. Coordination and/or assistance in crisis management and stabilization as needed;
7. Advocacy and outreach;
8. As appropriate to the care provided to individuals, and when appropriate, to the family, education and training specific to the individual's assessed needs, abilities and readiness to learn;
9. Mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment; and
10. Activities that increase the individual's capacity to positively impact his/her own environment.
11. The methods of CM service delivery shall consist of:
12. CM service provider shall have a staff development plan based upon identified individual needs of CM staff. Maintain and promote a healthy lifestyle and adhere to productivity requirements and procedures as defined by management staff.
13. Assist clients in scheduling appointments including medical, legal, Human Services, Social Security, etc.
14. Assist in activities for recreation, community integration and involvement;
15. Assist the clients who can work in obtaining employment in the area, and seek out new employment opportunities for the residents;
16. Coordination or assistance with crisis intervention and stabilization services.
17. Monitor the need for prescription refills.
18. Maintain up to date client records with all appropriate documentation including billing documentation in accordance with agency policies and procedures;
19. Perform drug testing and documentation, when indicated;
20. Assist in discharge planning, including linkage to other services (housing, outpatient treatment, etc.)
21. Assisting with accessing transportation – Supervise and assist in the transporting of clients to and from (Agency) programs for meetings, appointments, work, etc.
22. Other duties as assigned.

ADDITIONAL DUTIES AND RESPONSIBILITIES:
1. Community Psychiatric Supportive Treatment (CM) Specialists may be expected to work some evenings. They must be able to work independently with little supervision. Exposure to high stress situations with a diverse population of clientele.
2. Performs other duties as deemed as necessary and appropriate by supervisor.
REQUIRED QUALIFICATIONS:
1. College training or equivalent in work experience.
2. Training and experience in the mental health and addiction field.
3. Demonstrated working knowledge mental health, chemical dependency and recovery.
4. Demonstrated understanding of the continuum of client care and the transition/placement in the various phases of treatment.
5. Demonstrated working knowledge of client confidentiality and client rights.
6. Demonstrated computer proficiency and literacy.
7. CPR and First Aid trained and crisis intervention (CPI) trained.
8. Valid driver’s license and insurance.
PHYSICAL DEMANDS:
