Coordinates the total care of clients through written programs, observations, monitoring, and facilitating with various professionals (doctor, psychiatrist, etc.). 


Responsibilities

Monitor and coordinate the total care of the assigned caseload.

Communicate with family members at least monthly. Maintain positive relationships with families.

Prepare/coordinate reports for all regulatory compliance, e.g. monthly reports, evaluations, current plans, data sheets, medical information, etc. in a timely fashion; and maintain a comprehensive file.

Gather data, organize, write and implement each ISP in a timely fashion.

Give attention to requests of every person in the caseload, and follow through to him/her with a response. Also responsible for repeatedly letting each person knows that the staff exists to serve each person in the home/apartment.

Organize and participate in IDT meetings for updating and revising an ISP as needed in a timely fashion.

Monitor client supervisors and work with each member of the staff in implementing each ISP.

Meet with home/apartment manager as needed to gain assistance in client care.

Accompany clients at all psychiatric and psychological appointments.

Investigate all incidents/accidents involving clients.

Design and maintain a flexible schedule as dictated by the needs of the people receiving services.

Cooperate with and support other members of the team.

Attend in-service training sessions.

Provide hands-on assistance in transportation, running of client programs, appointments, and leisure activities as needed.

Demonstrate and maintain in-service training competencies.

Sustain a positive work attitude with regard to residents and staff.

Serve as Person on Call for seven day period on a rotating basis with other personnel within the department.

Successfully complete Certified Medication Administration Training and First Aid Training.

Staff direct support workers (DSWs) in accordance with consumer’s budgeted hours as indicated on the approved Comprehensive Plan of Care (CPOC). Recruit backup staff for callouts, emergencies, etc.

Revise and update staff schedules as needed. Distribute staffing schedules to the direct support workers (DSWs). Provide regular updates and feedback to the Director of Community Services.

Recruit, interview and hire direct support workers (DSWs) as per the consumer’s needs and in accordance with the consumer’s budgeted hours as indicated on the approved Comprehensive Plan of Care (CPOC).

Facilitate pre-employment screenings.

Must maintain an active account at a financial institution in order for pay to be direct deposited.

Must have access to a smart phone or electronic tablet with data plan to accurately capture time worked with clients.


Qualifications

Bachelor of Arts degree in social services field or three years of verifiable experience in mental health field.

Any related classes to mental health/developmental disability field helpful (for example, Behavior Management techniques).

Minimum of one year’s experience in mental health field.

Excellent reading, writing and math skills.

Must possess a valid Louisiana Driver’s License.

Must meet the minimum age requirement for automobile liability insurance.

Strong interpersonal skills.

Ability to perform duties with little supervision.
