Total # of Facility Beds: 98
Unit Name and Cost Center: LONG TERM CARE (OTTER CREEK EAST & WEST)
CNA for LTC: Floating required. Cat on unit - cannot be allergic. Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review. Travelers must work 2 out of the 4 holidays: Thanksgiving, Christmas, Christmas Eve, and New Years Day.
Requirements:
• Prior travel required
• 2 years of experience required
• Long term care experience preferred
• Questionnaire attached to position files must be filled out and added to sub packet
• Epic experience from within the past 2 years is required and nonnegotiable
Additional Information:
• IVS to OFFER
• Monday to Sunday
• No block schedules for any reason
• Bi-weekly orientations on Monday, next available is 9/01
• Please ensure all employment gaps over 90 days are explained
• This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit
