Job Details
The Family Support Specialist functions as a member of the Pediatric Integrated Health (PIH) Care Team providing direct and indirect care coordination services to clients and families. In addition, this position supports transitional living processes for clients. Through an understanding of community based resources, the PIH Family Support Specialist will assist clients and families in locating and accessing social, community and recovery supports.
Qualifications
Qualified Candidates
- High School Diploma or GED.
- A minimum of one (1) year of experience working directly with children with mental health and/or physical health needs.
- Any equivalent combination of experience and training which provides the knowledge and abilities necessary to perform the work Required Special Qualifications.
Required Special Qualification
- Must have a valid Driver's License and meet the Agency's driving requirements.
Salary : $16 - $18
