What are the responsibilities and job description for the Medical Social Worker/Case Manager position at Team Nurses Home Health Services, Inc.?
Job Summary: Upon orders of the physician or poatrist identifies factors which influence a patient's and/or family's ability to respond to the plan of care. Through consultation and home visits, develops means to meet the goals of the plan of care. Provides counseling, as appropriate.
Qualifications:
A Masters of Social Work Degree, LCSW from a school of social work accredited or approved by the Council on Social Work Education or a related field is preferred. Titles next to signature are to include the appropriate degree attained: MSW or LCSW.
One year of social work experience in a health care setting preferred.
BSW and SWA employees work under the supervisio of an MSW (Master of Social Work) or LCSW (Licensed Clinical Social Worker)
