Habilitation Coordinator - Intellectual Developmental Disability (IDD)


Role Responsibilities

The Habilitation Coordinator manages services for participants in one of the following settings: nursing facility or community. In the nursing facility, the Habilitation Coordinator is responsible for PASRR duties. In community settings, the Habilitation Coordinator is responsible for performing enhanced community coordination (ECC) duties for individuals transitioning or diverting from institutional care. The Habilitation Coordinator is responsible for the following:

  • Assessment, service planning, monitoring, advocacy and crisis prevention and management for assigned caseload of individuals with IDD.
  • Provision of Medicaid billable Habilitation Coordination activities for individuals in a nursing facility.
  • Provision of Medicaid billable targeted case management activities for individuals in a nursing facility transitioning to the community.
  • On call response to PASRR Evaluation alerts within 72 hours.
  • Compliance with applicable local, state and federal rules and regulations.
  • Adherence to and promotion of the Texana Compliance Policy.
  • Travel within 6 county region.
  • Work hours: M-F, 8-5 with flexibility based on client need.

Salary

  • $21.69/hour - $25.25/hour based on years of experience

Required Education and/or Certifications, Experience, Skills

  • Bachelor degree in a social, behavioral, human services field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.
  • Texas Driver License (in good standing)
  • Habilitation Coordinator I: One year of experience as Service Coordinator serving individuals with complex needs required.
  • Habilitation Coordinator II: Three years as Service Coordinator serving individuals with complex needs, or equivalent capacity with IDD Community Center or State Facility.
  • Habilitation Coordinator III: Five years as Service Coordinator serving individuals with complex needs, or equivalent capacity with IDD Community Center or State Facility.
  • Habilitation Coordinator IV: Seven years as Service Coordinator serving individuals with complex needs, or equivalent capacity with IDD Community Center or State Facility.

Preferred Education and/or Certifications, Experience, Skills

  • Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).
  • Knowledge of State Rules relevant to Habilitation Coordination, Service Coordination and PASRR.
  • Experience working with individuals with IDD with complex medical and/or behavioral health needs.

Job related Physical Requirements

  • Must be able to operate a vehicle
  • Must be able to perform SAMA and CPR. Training is provided to employees once hired
  • Must be able to perform moderate to heavy lifting (15#- 45#), pushing, stooping, light carrying (15#), repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting
  • Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
  • Qualified individual must be able to perform these functions with or without reasonable accommodations
