Registered Nurse (RN)- Assertive Community Treatment (ACT) Team
Role Responsibilities
Under the general direction of ACT Clinical Team Lead, the ACT RN is responsible for providing nursing care to all individuals assigned to the ACT team, ranging between 70 and 100 clients. The position provides nursing care, Psychosocial Rehab Services, Medication Training and Supports to individuals with a mental illness and/or a co-occurring substance use/abuse diagnosis along with individuals with a dual diagnosis of mental illness and intellectual or developmental disability. Duties include delivering nursing care, assessments, recovery planning, and coordinating nursing activities as a member of the interdisciplinary team. Utilizes the nursing process to plan, implement, and document and evaluate nursing care provided to patients under the requirements of the Texas Nurse Practice Act. The ACT RN provides lab and medication related services in vivo to ACT clients in the community. The ACT RN documents assessments and billable services within the electronic health record within the appropriate time frames. Working hours are Monday – Friday 8am-5pm with some flexibility needed to meet program needs. Performs tasks as assigned by Clinical Team Lead or Clinical Support Manager. Responsible and accountable for the adherence to and promotion of the compliance policy. Must be able to work within a multi-disciplinary treatment team. Must have excellent engagement, time management and organizational skills.
Salary
- $86,100.04 / annually
Required Education and/or Certifications, Experience, Skills
- RN License in good standing
- Texas Driver’s License in good standing
Preferred Education and/or Certifications, Experience, Skills
- 1 year experience working with adults with serious mental illness
- Current CPR
- Experience in phlebotomy/EKG procedures
- Basic computer skills
- Bilingual- Arabic, Mandarin, Spanish, Vietnamese
Job Related Physical Requirements
- Must be able to perform SAMA and CPR
- Must be able to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
- Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
