We are currently hiring for Supports Coordinators to work with individuals of all ages with intellectual and developmental disabilities . The position provides case management services to a group of individuals with intellectual disabilities living in various community living arrangements programs or their own/family homes throughout Berks, Bucks Chester, Delaware, Montgomery and Philadelphia counties. The Supports Coordinator is responsible for monitoring their health and safety and providing follow up support when needed. Their job is to locate and coordinator services and supports for these individuals. If it is through their Medicaid waiver through the state or accessing community and natural resources available.
The Supports Coordinator‘s position involves the location, coordination, and monitoring of needed services and supports for people with intellectual disabilities. Caseload areas: Chester, Delaware, Montgomery and Philadelphia counties.
The Supports Coordinator is a liaison between funding sources and the individuals that need them.
The Supports Coordinator is a team leader who assists the individual and family in navigating supports available to the individual.
The Supports Coordinator assists participants in obtaining and coordinating needed waiver and other services to ensure health and safety
The Supports Coordinator locates services through various providers and in the community
The Supports Coordinator coordinates services to best suit the needs of the individual
The Supports Coordinator monitors services to ensure the health and safety of the individual
The Supports Coordinator works with the individual and their team to realize the rights to which they are entitled, to obtain needed services, and remove barriers to full participation in their communities.
The Supports Coordinator must be organized, have excellent communication and time management skills, the ability to work effectively with people with disabilities, to aid them to meet outcomes based on their needs.
Caseload areas: Chester, Delaware, Montgomery and Philadelphia counties - This position requires travel to participants’ homes and community locations.
Bachelor’s degree from an accredited college or university which is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social sciences. Must present a clean PA Criminal background check, PA Child Abuse and FBI Criminal Record Check at time of hire.
