Job Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to clients in need of assistance. This role requires excellent organizational skills and the ability to manage multiple tasks efficiently.
Responsibilities:
- Conduct intake assessments
- Coordinate services and resources for clients, including medical appointments, social services, and community programs
- Maintain accurate and up-to-date client records and documentation
- Monitor client progress and adjust care plans as needed
- Communicate effectively with clients, families, and other professionals involved in the case
- Provide crisis intervention and support as required
Requirements:
-experience in the addiction field is a must
- Strong organizational skills with attention to detail
- Excellent communication and interpersonal abilities
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
- Experience with case management software is a plus
- Knowledge of medical office management practices
- Ability to maintain confidentiality and handle sensitive information appropriately
- Bachelor's degree in Social Work, Psychology, or related field preferred
