Reports to: Cradles Program Manager
Hours: Full time, with some weekend and evening hours
Travel: Locally – Required (Mileage Reimbursement Provided)
Status: Non-Exempt
The Council on Recovery is a 501(c)(3) not-for-profit organization whose mission is to keep our community healthy, productive and safe by providing services and information to all who may be adversely affected by alcohol, drugs, and related issues.

The Role
The Outreach Case Manager (OCM) provides community-based intervention, outreach services and evidence-based education to parents with children under the age of six OR expecting parents who have one or more risk factors for substance use.
The OCM works one-on-one with parents to evaluate needs and risks, develop individualized strength-based service plans, offer case management services, and create space for families to engage in fun, healthy, sober free experiences.

Requirements
EDUCATION AND REQUIREMENTS
· Bachelor’s degree in Social Work or other related field, or· LCDC-I or LCDC (strongly preferred)
· Minimum of two years in the field of Behavioral Health, to include substance use experience
· Minimum of two years’ case management experience
· Previous experience working with vulnerable families/teens in the substance use field
· Working knowledge of client/community resources and experience in developing partnerships
· Experience leading groups with vulnerable populations
· Experience working independently and collaboratively as part of a team· Valid driver’s license, automobile insurance, and reliable transportation
· Bilingual (preferred)
