St. Joseph Villa Homecare and Hospice is seeking an Intake Coordinator to join our team. Home Health Intake experience is preferred, but not required. Proficiency in Microsoft Office Suite a plus.
Job Responsibilities include :
- Manages referrals throughout the entire intake process, including retrieving referrals from the fax or email server and expediting the processing of medical, social, and financial in the electronic medical record.
- Ensures that all OASIS and plan of care document is sent to the appropriate physicians, tracks the receipt of forms by working collaboratively with internal staff and external customers, and ensures that forms are returned and processed in a manner that meets state and federal regulations.
- Works cooperatively with the clinical department in providing continuity of information for patients and staff.
- Scans admissions documentation into the electronic medical record.
- Process financial and demographic information through appropriate Medicare and / or Medicaid systems to determine eligibility.
