Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent, affordable, and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. We are seeking a Senior Case Manager for one of our supportive housing programs. The Senior Case Manager is responsible for providing supportive case management services to Peter Jay Sharp residents, including assistance with obtaining public benefits, monitoring residents’ rent payments, developing independent living skills and socialization skills, and facilitating groups and recreational activities. RESPONSIBILITIES:Maintain a caseload and provide regular case management services to these individualsProvide counseling and crisis intervention as necessaryAssist appropriate residents in accessing and maintaining benefitsMonitor and assist residents with rent payments and budgetingMaintain resident records and complete a variety of charting documentation to ensure compliance with the DHS contractDevelop and implement individual service plans for each resident and assist the residents with achieving their goals geared towards independenceMaintain communication with the Property Management department and address any maintenance issues, as well as rental arrearsDevelop and facilitate support groups and recreational activities for the residentsMaintain relationships with community treatment providers, including substance abuse treatment programs, mental health programs, and hospitalsAttend ongoing training to enhance the services provided to the residentsOther duties as assignedQUALIFICATIONS:Bachelor’s Degree in Social Work or related fieldExperience working with homeless individuals and/or people with histories of substance abuse, preferably in a residential settingFamiliarity with policies and procedures of HUD Shelter Plus Care or DHS is a plusStrong organizational, writing, and communication skillsFamiliarity with psychiatric diagnoses and substance abuseSolid computer skills – internet, Microsoft Office, and charting database • Bilingual (Spanish/English) is strongly preferredInterested?
