The Excalibur Group is currently seeking a Support Broker/Case Manager in Washington, DC to support a local contract. This position is hybrid work with regular customer-site visits.


**Will be subject to a background check prior to employment.

SUMMARY
The case manager is responsible for coordinating the daily operation of Medicaid Waiver services, and for promoting personal growth and independence of participants in accordance with state and program regulations. This includes assisting the person to access and maintain all public benefits to which they may be entitled. The case manager’s role is to support the person in developing a written comprehensive person-centered plan for Medicaid and non-Medicaid services (including community resources) that reflects the person’s strengths, interests, preferences, community and family supports, personal goals, financial resources, and assessed needs. Case Managers are responsible for coordination of all local government provided waiver services to their customers, including participant directed services, to ensure that services are delivered in a well-coordinated, safe, timely, and cost-effective manner.

DUTIES AND RESPONSIBILITIES
  • Ensure personal growth and independence of service participants through initial development and on-going support, monitoring, supervision, and training of staff.
  • Develop and maintain relationships with Agency staff, other providers and individuals/families.
  • Know, understand, and abide by Agency's Mission and Values and quality of service standards.
  • Ensure quality of all documentation in accordance with all state and agency requirements.
  • Abide by all agency policies and procedures.
  • Monitor for and participate in performance improvement and ensuring quality in the health and safety of service participants.
  • Complete written assignments as assigned.
  • Assist in forming and sustaining a full range of relationships with natural and community supports that allows the participant meaningful community integration and inclusion.
  • Assist in locating and accessing non-Medicaid community supports and resources that are related to achieving Individual Support Plan (ISP) outcomes: this includes social and educational resources, as well as natural supports.
  • Provide instruction and counseling which guides the participant in problem solving and decision making.
  • Advocate and collaborate with other individuals and organizations on behalf of the participant.
  • Support the person in preparing, participating in, and implementing plans of any type (IEP, ISP, or service plan, etc.)
  • Provide training on the Individual and Family Directed supports Option, if the participant is considering directing services and supports.
  • Provide guidance with the management of the individual & Family directed budget.
  • Coordinate services with the financial Support Services provider, if the individual is self-directing services under the Employer of Record Model, including guidance on use of the Individual and Family Directed (Participant-Directed Budget).
  • Provide information on recruiting, hiring, managing, training, evaluating, and changing support staff, if the participant is self-directing services. 
  • Assist with the development of schedules and outlining staff duties, if the participant is self-directing services considering directing services and supports.
  • Assist with understanding staff financial forms, qualifications and record keeping requirements, if the participant is self-directing services.
  • Provide on-going information to assure that participants and their families/representatives understand the responsibilities involved with participant direction, including reporting on expenditures and other relevant information and training (Employer of Record Model).
  • Inform and coordinate community resources including coordination among primary, preventative and chronic care providers.
  • Assist in locating options for renting or purchasing a personal residence, assist with purchasing furnishings for the personal residence.
  • Other duties as assigned.
REQUIREMENTS
  • Must live in the DMV Area, but preference given to DC residents
  • Bilingual in Spanish or Amharic is preferred. 
  • Be 18 years of age or older. 
  • Bachelor’s degree and two (2) years’ experience with community outreach programs; OR a Master’s degree in social work, psychology, counseling, rehabilitation, nursing gerontology or sociology and one (1) year experience; OR Registered Nurse (RN); or can have an Associate’s degree and three (3) years’ experience with the community.
  • Successfully complete a criminal background check
PAY:
$55k

HOURS OF OPERATION
Mon. – Fri. 8am – 5pm
Except Federal Holidays
