A Typical Day in the Life…
A typical day as The Family Partnership’s Promising Practices Home Visitor provides an opportunity to support our Promising Practices program, which is designed to buffer young children and their caregivers from the impact of adverse childhood experiences (ACEs) and toxic stress. The program also supports school readiness and supports caregivers with children 3-5 years old to develop economic self-sufficiency skills. These activities will occur primarily in the family’s home, with the interventions being family-centered and strength-based. Responsibilities will include the following:
- Collaborating with TFP preschool family workers to identify preschool families enrolled and newly enrolled who qualify for Promising Practices.
- Meeting with families in the home weekly for at least one hour at each visit.
- Establishing a trusting relationship with families using a trauma-informed approach.
- Helping caregivers identify personal strengths and set personal and family goals.
- Assisting caregivers with learning and supporting their preschoolers with building executive functioning skills through weekly activity modules and children’s books.
- Providing strength-based, family-centered interventions and prevention strategies during weekly home visits.
- Assisting caregivers with identifying ways to increase monthly income, optimize their home environments, and improve their family support systems.
- Increasing families’ ability to problem solve and assume the role of advocate for themselves and their children.
- Referring, advocating, and following-up with clients in need of additional community support and services.
- Documenting all interactions in the electronic health record, including but not limited to case notes, assessments, intakes, surveys, discharge summaries, transfer summaries, and evaluations.
Who You Are
- You hold a Bachelor’s degree in human services, social work, education, or a related field.
- You have a minimum of two years of directly related work experience.
- You have experience working with families in home and/or community settings.
- You have experience working with and an understanding of communities living in poverty, communities of color, gay, lesbian, bisexual, and transgender individuals, and other vulnerable populations.
- You have excellent verbal and written communication skills.
- You have the ability to provide home-visiting services including traveling to clients’ homes, schools, meetings with community partners, and other various locations.
- You have access to reliable transportation and can transport clients in your personal vehicle.
- You have the ability to develop and maintain positive and respectful relationships with clients.
- You have the ability to track and maintain data necessary for program monitoring and evaluation.
- You have the ability to perform assessments as needed in order to conduct appropriate intervention and referrals.
- You have the ability to work respectfully and communicate effectively with different cultures, groups, and languages.
- You have the ability to work some evenings as required.
- You have a valid driver’s license, access to reliable transportation, and the ability to meet TFP’s driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
