Certified Occupational Therapy Assistant (COTA) – PRNFacility: The Gardens at StevensLocation: Denver, PAAbout the RoleThe Gardens at Stevens is seeking a Certified Occupational Therapy Assistant (COTA) to provide PRN coverage as part of our dedicated in-house therapy team. This role offers flexibility while allowing you to make a meaningful impact on residents’ functional independence and quality of life in a skilled nursing environment.Key ResponsibilitiesAssist the Occupational Therapist with resident evaluations and data collection for Section GG during Annual, Quarterly, Falls, and Significant Change assessmentsImplement individualized treatment plans and provide Activities of Daily Living (ADL) training under the direction of the Occupational TherapistMonitor resident performance and promptly report progress, changes, or concerns to the supervising OTMaintain accurate, timely documentation of treatments and outcomes in EMRs (Net Health/PCC) in compliance with regulatory standardsEducate residents, families, and staff on safe mobility techniques and therapeutic exercisesConduct Remote Therapeutic Monitoring (RTM) check-ins with discharged residents transitioning to home or other levels of careCollaborate with Physical Therapists, Speech-Language Pathologists, and interdisciplinary rehabilitation team members to deliver comprehensive, coordinated careQualificationsCurrent Pennsylvania COTA license in good standingPrior experience in a skilled nursing or long-term care setting preferredStrong knowledge of occupational therapy treatment techniques and rehabilitation practicesExperience with Net Health/PCC electronic documentation systems preferredWhy Join The Gardens at Stevens?Flexible PRN Schedule – Work assignments that fit your availabilityCollaborative In-House Therapy Team – Supportive, resident-focused environmentCompetitive Compensation – Based on experience and expertiseIf you are passionate about rehabilitation and committed to helping residents achieve greater independence, we encourage you to apply today and join a team that values your skills and dedication.
