What are the responsibilities and job description for the Blended Case Manager position at The Guidance Center?
Be part of a dynamic team to provide strength-based community case management services to individuals who possess a mental health diagnosis.
Duties include; development of recovery-oriented treatment plans, collaboration with assigned treatment team, link with community-based resources and services, provide advocacy and support individual recovery journeys, and coordinate with crisis services through an on-call rotation.
Work Location: Flexible
Work Remotely: Hybrid
Education Requirements: Bachelor’s Degree in Psychology, Sociology, Human Services, Criminal Justice, or related field.
Job Type: Full-time
Pay: From $16.00 per hour
