Community Care Coordinator
SUMMARY: The Community Care Coordinator is responsible for assisting residents with their Activities of Daily Living (ADL’s) and ensuring safe transport as needed to meals, activities and other resident needs. This position helps supervise and coordinate the schedules of the CNAs/Caregivers Team. The Community Care Coordinator works closely with other Caregivers towards overall resident care under the direct supervision of the Director of Healthcare.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Makes and monitors the schedule for all CNA/Caregiver and Medication Tech Staff and fills in when the need presents itself.
- Communicates to resident’s families and staff in collaboration with the Director of Healthcare and the Executive Director.
- Responsible for Med Cart Audits.
- Present information to the Director of Healthcare as to changes in condition or recommendations as it relates to Plans of Care.
- Attend meetings as required by the Leadership Team.
- Review all new admission charts for state and community required documentation.
- Respond to resident emergencies.
- Maintain resident TENA requirements and nursing supplies by coordinating an inventory process and communicating community needs to the Director of Healthcare and the Executive Director.
- Rounds on all assigned residents, assists with ADL’s as warranted in the Residents Plan of Care.
- Communicates with caregivers, medication tech’s, other department staff.
- Keeps resident files updated and organized in the health record, EMAR and other software programs.
- Assists residents with Activities of Daily Living (ADL’s).
- Opens or closes blinds per resident preferences, assists residents with shower, assists resident with dressing (weather appropriate clothing), Assists residents with hair, teeth brushing, removing and replacing dentures, putting on shoes/socks.
- Places dirty clothes in hamper
- Wipe down dining room table and kitchen counters and wash any dishes and place out to dry, return to cabinets when dry.
- Offer water or other drink to residents and ask for additional needs. Provide requested needs.
- As necessary assist resident with toileting or perform perineal care as needed for incontinence.
- Take out trash and pick up apartment.
- Take resident out for walk or to activities as requested.
- Straighten residents room and closets as needed.
- Answer call requests, respond to emergencies as needed.
- Report to Director of Healthcare any changes in the resident’s behavior, physical or mental status.
- Document all required information in the medical record and on community forms.
- Supplement hospice care as warranted in the resident plan of care.
- Completes incident reports for any incidents occurred on assigned shift.
- Supports resident care plan
- Facilitates and/or participates in resident group activities and resident care conferences.
- Engages residents in meaningful conversation and activities related to their individual interests.
- Maintains professional approach to responsibilities (uniform, start times, absenteeism, confidentiality, communication with residents, family members, other staff members and vendors, and daily work responsibilities).
- Supports new resident welcome and assimilation process.
- Monitors residents for changes in condition and reports changes to delegating nurse.
- Assists with resident room, bathroom, and common area cleanliness, and other housekeeping duties as needed.
- Performs other duties as assigned.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.
- High school diploma or equivalent.
- CPR/First Aid Certification
- CNA Certification
- Willing to be a delegated Medication Technician
- Complete 12 hours of continued education courses annually – provided through community in-services.
- Must be fluent in English and have the ability to read and interpret documents such as resident charts, medication orders, and safety rules, operating procedures, policies and employee manuals. Ability to write reports and correspondence.
