JOB TITLE: Licensed Practice Nurse (LPN)
STATUS: Non-Exempt
REPORTS TO THE RN SUPERVISOR
JOB PURPOSE: To ensure the efficient day-to-day operation of the nursing program for those clients utilizing Agency Directed Services, and support the work of the Certified Manager and other staff. Maintain and support the vision, mission and values of The Independent Living Center (TILC).
PRIMARY DUTIES AND RESPONSIBILITIES:
DIRECT SERVICES:
- Provide the highest quality of customer service consistent with TILC’s values and standards
- Utilize the nursing process to assess, evaluate, plan and implement client care which may include new Admissions; General Health Assessments; on site 10% visits; adequacy of the Plan of Care in meeting the needs of the client
- May fill (1) week supply of insulin syringes; set up oral medications; monitor skin conditions; provide nail care; and any other nursing services authorized by DHSS
- Evaluate/Assess the Attendant relative to his/her ability to carry out the Plan of Care
- Inform the Certified Manager of changes in client condition, or the need for changes in client Plan of Care
- Maintain an open communication/collaboration with other client providers, including the client’s physician, as directed by the client
- Other duties as assigned.
DOCUMENTATION:
- Maintain an accurate list of clients served on The Independent Living’s Network shared drive
- Maintain a monthly visit history to be documented for billing purposes
- Provide complete documentation of all admissions performed for the Registered Nurse to review
- Maintain a current medication record for those clients receiving medication set up
- Provide documentation to the Registered Nurse to review for all on-site visits
- Provide mileage reports to the Certified Manager when applicable
EDUCATION AND TRAINING:
- Obtain a working knowledge of Missouri state regulations regarding Agency Directed Service requirements
- Responsible for educating the client and family of disease processes, prevention and management; medication administration; and signs & symptoms requiring immediate contact of their medical provider
- Assist in the development and implementation of education and training for ancillary personnel with the exception of Advanced Personal Care training
- Participate in Attendant meeting as requested by the Certified Manager
- Participate in orientation of newly hired Licensed Practical Nurses
QUALIFICATIONS:
- Graduated from an approved school of training
- Maintain current licensure from the Missouri State Board of Nursing
- Maintain a current CPR certification
- Maintain a valid Missouri Driver’s License
- Maintain proof of valid automobile insurance
- Possess the ability to work independently, to establish priorities, and have excellent organizational skills.
- Ability to communicate and interact with persons from varying cultures and backgrounds and education
- Ability to push, pull, lift, or carry up to 25 pounds
