Job Summary
We are seeking a dedicated Nurse Practitioner to join our team in a unique grant-funded role supporting Jewel Human Services through deployment at the ROCK Community Center and CARE Center in Indianapolis. This role is part of the HF MCPHD grant and supports the delivery of high-quality, integrated care to underserved communities in non-traditional settings. The Nurse Practitioner will operate under JPCHCs clinical standards while delivering mobile and community-based services in alignment with grant objectives.
This position is expected to remain active through the duration of the grant period ending (enter date). Continuation beyond that date is contingent upon renewal or extension of grant funding and HRSA scope approval.
Job Responsibilities (include but are not limited to):
- Provide direct patient care including physicals, immunizations, PCP visits, and referrals.
- Deliver services at both the ROCK and CARE Centers, utilizing onsite nurses stations and clinical resources.
- Adhere to JPCHCs clinical standards, preventive care protocols, and wellness service guidelines.
- Coordinate care across departments and refer patients for services not available on site.
- Collaborate with other healthcare professionals to meet the unique needs of each patient.
- Contribute to the success of grant-funded objectives and ensure compliance with all related requirements.
- Participate in the provision of mobile and/or home-based services as defined by the grant.
- Maintain thorough and timely clinical documentation in accordance with FQHC and HRSA standards.
- Ensure alignment with HRSAs approved scope of project, including any required 5C Form changes for service delivery sites.
Required Skills and Qualifications
- Licensed Nurse Practitioner in the state of Indiana.
- Certification by a nationally recognized NP certifying body (e.g., AANP, ANCC).
- Minimum of one (1) year experience as a practicing NP, preferably in community or primary care.
- Strong clinical judgment, flexibility, and ability to work in both traditional and mobile settings.
- Experience working with underserved populations and/or grant-funded programs preferred.
- Excellent communication, collaboration, and problem-solving skills.
- Commitment to culturally competent, trauma-informed care.
