What are the responsibilities and job description for the CASE MANAGER position at The Midnight Mission?
The Counselor/Case Manager will be responsible for counseling, crisis intervention, client outreach, intake, and case management.
PRIMARY DUTIES
· Oversees and guides the rehabilitation and transition plans for Program Participants.
· Conducts individual and group counseling sessions.
· Prepares and updates case file notes and reports on program clients’ progress
· Conducts program intake screening interviews, and recommends program placement or referrals
· Moderates discussions and workshops on anger management, conflict resolution, and life planning skills.
SECONDARY DUTIES
· Complete various special projects as directed.
· Supervises New Client detoxification process
· Produce various reports for staff, management and board of directors.
JOB REQUIREMENTS
· Valid Calif. Driver’s License & Social Security Card.
· Certification in substance abuse counseling
· Thorough knowledge of 12 Steps
· Must have a neat, clean professional appearance.
· Able to take initiative and work independently.
· Able to work well with clients, staff, volunteers, visitors and outside organizations.
· Exceptional oral and written communications skills.
· Exceptional organizational skills.
· Strong administrative skills.
· Must understand and embrace the vision of the Midnight Mission.
