Position Title: Admission Nurse
FLSA STATUS: Non-Exempt
REPORTS TO: Post Acute Unit Nurse Manager
SUPERVISES: None
DEPARTMENT: Health Center Nursing Services

POSITION SUMMARY:
The Admission Nurse implements the interdisciplinary resident care plan by providing excellent direct care.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
· Prior to the resident/patient arrival, review incoming hospital documentation from admission department for potential new patient.
· Review patient’s medical history and hospital discharge status for comparison at time of admission.
· Upon arrival greet and welcome resident/patient. Educate them on the Call don’t Fall program.
· Ensure signing of regulatory consents as appropriate.
· Initiate and work to complete the admission checklist.
· Perform assessments (skin, wound, elopement, etc.) as appropriate and establish aligning initial care plans.
· Resident/patient review and reconcile patient’s orders.
· Communicate with nursing, rehab services, social services, dietary, and pharmacy team.
· Communicate any appointments or schedule procedures that are occurring during post-acute stay.
· Ensure completion of the initial care plan and communicate to team members according to their licensure experience, and expertise.
· Initiate MDS section GG; 3 Day Assessment Period Data collection Tool, on day of admission for regulatory compliance.
· Adheres to facility policies and standards of care.
· Participates in care delivery by providing medications and treatments and other nursing tasks as assigned.
· Participates in the interdisciplinary care planning process as indicated.
· Documents care and files reports as required.
· Will participate in weekend nurse on-call rotation.
· Other duties and responsibilities that may become necessary or as directed by the unit manager or Director of Nurses Services.
QUALIFICATIONS:
· Must possess a current, unencumbered, active license to practice as an LPN or RN in Massachusetts - required
· Minimum of 2 years relevant nursing experience - required
· Current CPR certification - required
· Knowledge of standard nursing practice, state regulations and the Nurse Practice Act
