The PARC at Joliet is seeking qualified RN / LPN for our Post Acute Rehab Center!
Responsibilities and Duties
- Perform ongoing assessments / observations and communicate significant findings, changes to supervisor and MD.
- Patient Teaching and follow up documentation
- Follow / implement fall precautions as per plan of care
- Provide care to residents following physician orders / treatments
- Facilitate communication among residents / families and nursing staff
- Supervise CNA's and delegate tasks
- Utilize Point Click Care for documentation and e-MAR
- Demonstrate commitment, professional growth and competency.
- Attend meetings, in-services, workshops
Qualifications
Graduate of an accredited LPN / RN Program
Must be licensed as an RN / LPN in the state of Illinois
Current CPR
