JOB SUMMARY:
The Family Resource Navigator supports families and establishes and maintains communication with community resources to support positive relationships for families with children and ensure strong referral pathways to and from our programs. The Navigator establishes new community resource partners to strengthen referral pathways between agencies, works with families to ensure their needs are met, coordinates training and systems orientation opportunities, as well as distributes temporary emergency supplies until referral to other agencies/programs occur.
PRIMARY RESPONSIBILITIE
- Offer rapid response on site at the agency office or by phone within 48 to 72 hours when a family has requested assistance.
a. Listen and provide confidential emotional support.
b. Help families with goal setting.
c. Empower families to make informed choices that are right for them.
d. Assess families’ needs and strengths.
e. Collect assessment and referral information.
f. Provide peer support in one-on-one and group settings.
g. Role model and coach families how to identify and access the supports they need.
h. Refer and link families to community resources.
i. Help families understand the mental health evaluation, diagnosis, and treatment process.
j. Discuss options in an unbiased and culturally sensitive manner.
k. Explain processes, rights and responsibilities, and timelines for services.
l. Offer guidance with forms and paperwork.
m. Support transition, safety, and reentry planning.
n. Identify relevant benefits programs (e.g., Medicaid, SNAP); support enrollment as appropriate.
- Create and Liaison with a Parent Advisory Committee for the Family Resource Center, creating a forum for dialog, ongoing needs assessment to identify priority needs of families and continuous quality improvement
- Coordinate FRC meetings, workshops, and trainings (e.g., parent information programs, parent education, family health screenings, etc.) for the purpose of delivering services in compliance with established guidelines.
- Identify and nurture relationships with community resources, services and providers that can offer system support to families in a timely and appropriate manner as referred by PCCT.
- Responsible for the purchase and inventory of consumables and supplies to keep the FRC stocked and organized.
- Communicates with agency staff regarding Family Resource Center policies and procedures to promote quality referrals and services to families.
- In collaboration with Supervisor and Community Relations Manager, engage in school or community outreach and awareness events.
- Work with Supervisor and the Community Relations Manager to promote the Family Resource Center/PCCT by communicating with referral sources (e.g., ob/gyn practitioners, hospitals, pediatricians), communicating with groups that interact with families (e.g., employers, newspapers, churches, childcare agencies, the public), and soliciting support from community leaders (e.g. to publicize the program and to provide support for families).
- Attend trainings as required, to include, but not limited to, Standards of Quality for Family Strengthening & Support, Developing and Sustaining Effective PAC’s, and technical sessions.
- Demonstrates the agency’s values of compassion, respect, learning, adaptability, and achievement.
- Other duties as assigned by the Community Outreach & Education Manager.
Requirements
REQUIRED QUALIFICATIONS
- Bilingual in English and Spanish.
- Firsthand experience navigating systems on behalf of child, youth, or young adult with health challenges and working with families.
- Familiarity with the resources in your community and region.
- Understanding of relevant systems, services, and processes (e.g., TANF, Medicaid enrollment).
- Strong interpersonal skills and empathy.
- Understanding of and ability to maintain family confidentiality.
- Familiarity and ability to work effectively with families of diverse racial, ethnic, socioeconomic, and cultural (e.g., LGBTQ, military) backgrounds.
- Strong written and spoken communication skills, including familiarity with relevant languages for your region.
- Ability to collaborate and network with community organizations and groups.
- Active involvement in other community councils, advisory boards, or parent and family groups.
- Formal training and/or work experience within the social service/case management field or training specific to navigating community service systems).
- Ability to lift 25 lbs.
PREFERRED QUALIFICATIONS
- Bachelor’s degree in the social services or other relevant field.
- Experience in human services in the public sector, skill in interacting with human service providers and agency leaders, and an understanding of the role of evaluation in social services.
Salary : $40,000 - $50,000
