Description

Summary/Objective

The Project exists to eliminate the disparities in health experienced by LGBTQ persons, people living with or at risk of contracting HIV (Human Immunodeficiency Virus), or HCV (Hepatitis C) through education and the provision of services that promote health and wellness.

The Case Management Manager is responsible for the oversight of the direct service operations, fiscal, and personnel management of HIV case management services in the Quad Cities (Iowa and Illinois) and surrounding service areas. The Case Management Manager is a member of the Public Health Services Department and will provide supervision to the HIV Medical and Non-Medical Case Managers at TPQC.

  • Responsible for the direct supervision and performance of the HIV case management team to include hiring, performance review, and discipline.
  • Act as the subject matter expert for case management standards and allowable client assistance requests for Ryan White Part B Iowa and Ryan White Part Illinois to support case managers and fulfill grant expectations.
  • Performs monthly audits of client charts in accordance with grant-related expectations and audit-related scoring systems.
  • Maintain a small caseload of clients living with HIV/AIDS. Complete and monitor biopsychosocial assessments and service care goals for each client.
  • Manage daily operations for case management services including being on the frontline for client de-escalation and addressing any client and assigned case manager obstacles and/or client grievances in conjunction with the Director of Public Health Services.
  • Document and report performance issues for case management staff who are not performing at the required standard. Create and implement performance improvement plans and corrective action plans for case managers. Complete 6-month and yearly performance reviews with oversight from the Director of Public Health Services.
  • Oversee timely compliance of professional training, certification requirements, licensure updates, and any job specific requirements.
  • Monitor, evaluate, and provide technical assistance and training to ensure programmatic requirements are met.
  • Assist with administrative tasks required for the direct provision of the Iowa and Illinois Departments of Public Health Ryan White Part B services and funding. Participate in all site visits with IA-DPH and IL-DPH.
  • Ensure financial assistance provisions are allowable, reasonable, and within budget.
  • Maintain effective working relationships with TPQC referral agencies including Genesis Infectious Disease Clinic, Trinity UnityPoint, University of Iowa, and De La Cerda House.
  • Assign new referrals to the appropriate case manager based on caseload size, state of residence, and client acuity.
  • Oversee the training and onboarding of new case managers as well as maintaining records and compliance with grant funders for annual trainings and requirements.
  • Maintain knowledge of emerging needs, services, and evidence-based intervention and informational developments in relation to HIV care.
  • Oversee interdisciplinary care team meetings across care services departments with external medical and mental health providers.

Leadership

  • Provide agency leadership through full participation on the Executive Leadership Team, through involvement in agency-wide activities, and by acting as a role model for employees
  • Serve as a collaborative team member, viewing the operations of their department and interconnectedness of their team’s work in relationship with the entire agency
  • Supervises, mentors, and trains direct report staff. Provides continuous performance management and conducts employee performance evaluations. Addresses performance and/or behavior issues, clarifying expectations, and providing feedback.
  • Advance the mission of the Project by developing and maintaining professional relationships with individuals, organizations and corporate entities who support the mission or who have potential to support the mission
  • Represent the Project at organization events, community events, and meetings.

Supervisory Responsibility

This position has supervisory responsibility for the Case Management team, including 9.0 FTE. Supervision includes daily direction, guidance, coaching, discipline, performance appraisal and recommendation for increases to salary.


Requirements

Physical Demands

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work is normally performed in a typical interior/office work environment, with typical office noise and other disruption.
  • While performing the duties of this job, the employee is regularly required to talk or hear. Both standing and sitting are required.
  • Various types of equipment/supplies are used to accomplish the job requirements and include, but are not limited to, calculators, computer keyboards, telephone, printers, scanner, copier etc.
  • The employee may be required to lift and move up to 10 pounds and occasionally lift and move objects up to 25 pounds.

Position Type/Expected Hours of Work

Normal hours of work and days for TPQC are Monday through Friday 8:00am to 5:00pm but hours for this position may vary. Working flexibly (some days of the week remotely) is available.

Travel

Overnight travel may be required 4-8 days per year.

Required Education and Experience

  • Bachelor’s degree in Public Health or Social Work or related field such as psychology, behavioral health, etc. is required.
  • Five or more years of related experience or demonstrated knowledge, skills, and abilities to perform the job requirements.
  • Strong cultural awareness of people living with HIV/AIDS, men who have sex with men, communities of color, and people with substance use issues, and how this may affect or influence service delivery.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical conversations for how The Project can better meet the needs of those we serve.

Preferred Education and Experience

  • At least two years’ experience in grant and contract management particularly related to fiscal oversight, service implementation, ensuring deliverables are met, and reporting requirements are fulfilled.
  • Demonstrated commitment to advancing equity and inclusion in workplace or community settings

Licensure/Certification Requirements:

  • Valid driver’s license and current vehicle insurance coverage
