Identify and address the psychosocial needs of residents and their familiesPerform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errorsFormulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.
Include family in teaching as appropriate.Demonstrate knowledge of clinical social work and discharge planning.
Interpret and promote residents’ rights and the “Residents’ Bill of Rights”Interact professionally with resident and family and involve resident and family in the formation of the plan of care.Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.Be flexible, organized, and function under stressful situations.Maintain a good rapport and working relationship with medical staff, departments, and employees.Manage and operate equipment safely and correctly.Treat patients and their families with respect and dignity.Support and maintain a culture of safety and quality.Qualifications of Social Services Supervisor:Bachelor’s degree in social work is preferred or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychologyOne year of supervised social work experience in a health care setting working directly with individuals is preferredCurrent State Clinical Social Work LicensureAbility to work cooperatively as a member of a teamAbility to communicate effectively with residents and their family members, and at all levels of the organizationAbility to react decisively and quickly in emergency situationsAbility to maintain confidentialityAbility to read, write and speak the English languageKnowledge of training techniques for clinical staffGood Ethical PracticeRelationship managementEEO Statement:Our facility provides equal employment opportunities.
