The Redwoods was founded in 1972 and operates as a stand-alone care community that supports financial health for seniors by offering HUD financial assistance, Medicare rehabilitation, and Medi-Cal for long-term care.
Summary
The Director of Nursing ("DON") will be responsible to plan, organize, direct, control, and evaluate the nursing service in order to meet the total needs of the residents and maintain a quality of service that will fulfill the objectives of the facility and be in accord with the policies set forth by the facility Administration and Governing Body on a twenty-four (24) hour basis. Ensures all staffing and regulatory requirements are met. Primary responsibility is for resident care in the long-term call hall referred to as “The Meadows”. Works closely and assists as necessary the ADON responsible for ‘The Groves” short-term rehab, skilled nursing, and hospice services hall.
Responsibilities
The primary duty of the Director of Nursing deals with the organization and administration of the nursing services within the Health Care Center. This is centered around the actual care of the residents, including the processing of all reports; the selection, training, and supervising of licensed nurses, charge nurses, and nurses’ aides, and the review of the scheduling of personnel under his/her supervision in such a manner as to provide a maximum level of care and meet all regulatory requirements. Ensure staff are following procedures set forth for infection control.
Staffing and Leadership:
- Must recommend to the Administrator the number and levels of nursing personnel to be employed, participate in their recruitment and selection, training and supervising, and recommend disciplinary action or termination of employment when necessary.
- Ensures that required staffing levels are achieved on a daily basis. Presents plans and ideas for achieving proper staffing. Notifies the Administrator when staffing levels are not met and notifies Human Resources of any current or pending vacant positions.
- Develops a cohesive group of employees who work together and support each other. Displays the ability to positively influence/persuade staff.
- Hires and retains key staff members with the skills and abilities to achieve departmental vision, values, goals, and objectives. Ensure appropriate discipline and control.
- Ensures and encourages training and education for staff members (i.e., orientation, skills labs, resident-for-a-day, life skills, etc.).
- Provides good communication and ensures the dynamic flow of information between employees of all levels.
- Attend and participate in facility mandatory in-service training programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention, etc.).
- Attend and participate in workshops, seminars, etc., as approved. Stays current on best practices.
Resident Care:
- Participates in the development and implementation of the Resident Care Policy Committee.
- Participates in the development and implementation of the Quality Assurance Committee.
- Conducts morning stand-up meetings.
- Assists in the planning of orientation programs for new personnel and continuing in-service in that nursing care plans are established for each resident and that the plan is reviewed and modified when necessary.
- Works closely with admissions and the ADON to ensure that the facility budgeted census is achieved or exceeded.
- Remains constantly alert for changes in resident’s symptoms and notifies the physician in charge when warranted.
- Sees that the physicians are notified immediately of the arrival of new residents, that the residents are assigned a bed, and institute orders and general nursing care according to prescribed instructions and ordered tests. Ensures charts and documentation are completed and in order, working in close cooperation and coordination with other departments within the facility.
- Oversees the nursing notes to ensure they are complete and correct and following regulatory and nursing guidelines.
- Works closely with the Medical Director and attending physicians and other medical and health-related personnel.
Medication and Treatment Administration:
- Oversees medication administration with the nurses ensuring minimal to no medication errors.
- Works closely with the pharmacy and pharmacy consultant.
- Monitors the use of medication for timelines, correct dosage, and administration. Ensures that all expired and discontinued meds are disposed of properly.
- Monitors resident rooms to make sure any allowable medications are clearly labeled and stored properly with the proper documentation in the resident medical record.
Reporting:
- Ensures all incident reports are complete and promptly made out, presented to the Administrator and/or HR for signature, and submitted promptly to the proper agency as required.
- Conducts incident and accident investigations as per policy and provides proper follow-up and communication to key leaders and government agencies as required.
- Submits all required nursing-related reports timely and to the proper agencies or staff.
- Contacts the county or state as required to report disruption in services and emergencies
- Writes or assists in writing the CDPH plans of correction and ensures they are followed up and completed as stated.
Quality of Nursing Service:
- Provides nursing care for residents of high quality and that meets their needs. Works with appropriate personnel to assure the nursing department provides a comprehensive and caring experience.
- Maintains contact with family members
- Maintains and routinely audits control (i.e., care plans, MDS, skin, census, drugs, supplies, safety, and infection control systems)
- Proactively works with external and internal resources to continuously evaluate and improve the quality of the nursing programs.
- Promotes an environment of innovation and creative approaches. Encourages others to try new ideas. Not only open to but sees the necessity of constant change.
- Conducts routine and frequent rounds. Interviews residents, families, and staff about the quality of service.
- Ensure proper documentation by all nursing personnel and monitor proper documentation by all interdisciplinary staff.
- Ensure Universal Precautions and Infection Control procedures are followed.
Business Practices:
- Controls expenses as per the budget. Manages variables (labor and supplies) to achieve planned financial targets; keeps expenses in line with budgets; responds timely to fluctuations in the census.
- Responsible for maintaining adequate control of stores and proper requisition of same.
- Maintains a two-week supply of PPE for COVID-19 response.
- Ensures that all resident supplies are available, distributed, and billed properly
- Ensures that all nursing related equipment is in good working condition
Resident Rights:
- Maintains the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
- Assure that residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are followed.
- Ensure resident’s valuables are inventoried and deposited or a “Release from Responsibility” form is signed in accordance with established policies and procedures.
- Provide resident/family with information concerning resident funds, resident rights, advance directives, etc.
Qualifications
- Must be a Registered Nurse in good standing and currently licensed by the State.
- Minimal (5) years of previous experience in health services for older adults. Skilled nursing facility management experience preferred.
- Must be able to relate positively to residents, families, and government regulators, and to work cooperatively with other associates at all levels. Excellent oral and written communication skills.
- Must have experience in performance management and effective leadership.
- Must have the ability to complete multiple projects.
- Must possess computer skills – MS Office applications, MatrixCare, etc.
- Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation including prioritizing, delegating and completing multiple projects.
