What are the responsibilities and job description for the Resident Care Coordinator position at The Residence at Village Green?
The Resident Care Coordinator provides direct care, supervises and trains the resident care staff, and assists with the facility’s marketing efforts. The coordinator communicates professionally with residents, families, and peers, and as part of a team, helps to create a positive, caring environment. The Resident Care Coordinator is accountable to the Executive Director. In the Executive Director’s absence, the Resident Care Coordinator may be expected to assume additional administrative responsibilities.
Qualifications:
- High school education or its equivalent, and/or certification in one of the following areas: Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or Licensed Social Worker (LSW).
- Must have previous experience in a care-related setting. CPR training helpful but not necessary. Experience with older adults preferred. Supervisory experience preferred. Current QMAP Certification. Knowledge of Word Processing and Excel Spreadsheets.
- Must have the ability to communicate clearly and effectively, be organized in the management of resident care and the supervision of staff, and have the strength and endurance required for standing, walking, bending, lifting and pushing.
Skills:
Resident Care
· Interview and orient new residents and their families.
· Evaluate the level of care required by each resident.
· Complete an individualized written assessment and plan of care for each resident.
· Monitor resident status and modify the plan of care as needed.
Staff Development and Supervision
· Provide orientation, training, supervision and evaluation for all personnel assigned to the Resident Care Department.
· Make daily rounds to monitor care provided by Resident Caregivers.
· Schedule staff hours for the Resident Care Department, and adjust as needed to maintain adequate staffing.
· Arrange mandatory in service training programs at regularly scheduled intervals.
· Coordinate with the Executive Director as needed for interview, hiring, disciplinary, or termination actions for Resident Care Department staff.
· Know all current state regulations governing resident care and medication administration, and monitor compliance.
Medications
· Ensure that Medication Administration Records (MARs) are maintained complete and up to date, coordinating with the Executive Director as needed.
· Monitor documentation of medications by the resident care staff, and compliance with internal medication policies and procedures, coordinating with the Executive Director as needed.
· Supervise medication assistance by the resident care staff, including compliance with State regulations.
· QMAP PRN
Communication
· Communicate with residents’ family members as needed, including but not limited to communicating changes in resident status, condition and care plans.
· Encourage open, professional communication among staff.
· Maintain all resident health chart forms and ensure all resident health charts are maintained accurately and completely, updating or archiving outdated material as needed.
· Monitor documentation of care by Resident Caregivers.
· Prepare paperwork required for resident transfers, discharges and physician visits.
· Document significant information accurately, completely, and in a timely manner.
· Coordinate care with physicians, pharmacists and other health care agencies.
Marketing
· Review resident health chart forms with potential residents and their families, explaining the resident care system.
· Make community visits to interview and evaluate potential residents, or existing residents desiring to return to the facility from a higher level of care.
Administration
· Maintain an adequate inventory of resident care supplies.
· Take measures to contain costs and stay within budget.
· Serve as Manager on Call on a rotating basis.
Guest Relations
· Demonstrate a courteous, polite, and friendly attitude with residents, families and visitors.
· Respond to the needs of residents, families and visitors by providing immediate assistance.
· Function as a leader of the resident care team, assisting co-workers as needed.
