The Salvation Army Midtown Service and Treatment Center, located in downtown St. Louis is looking for someone who is driven by wanting to help others achieve their goal of creating a better life for themselves.  We have an opening for a full-time, 40 hours per week, Housing Outreach Liaison. This position pays $45,000 per year.

 

 

Under the direct supervision of the Clinical Supervisor, The purpose of the Housing Outreach Liaison position is to: a) Reduce burden on current direct service staff to resolve housing crisis issues; b) Improve client outcomes due to an increase in agency skills to serve clients who experience multiple barriers to housing; c) Improve institutional knowledge around housing resources; d) Access specialized training and support; e) Leverage the existing strengths of the agency to implement effective outreach and engagement of clients experiencing housing crisis; and, f) Create a powerful network of experts to assist with resolving housing related problems in the community and statewide the Housing Outreach Liaison will proactively provide mental health and substance abuse assessment, intervention, treatment planning, counseling and referrals for clients and their families. This position will provide individual, group and family counseling and prevention services.  Educate staff and residents on crisis intervention, stress management, mental health, trauma and addiction.  Position is expected to comply with and meet The Salvation Army, Federal, State and Local policies and standards.

 

Essential Duties:

  1. Each Housing Outreach Liaison shall engage in housing related functions on a full time basis.
  2. Practice Housing First, harm reduction and other low-barrier service delivery models.
  3. Attend local Continuum of Care meetings related to coordinated entry/case conferencing.
  4. Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness.
  5. Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits.
  6. Landlord recruitment and rapport building. Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport.
  7. Receive referrals from Continuum of Care programs to assist individuals with housing stability.
  8. Obtain SSI/SSDI Outreach, Access, and Recovery (SOAR) certification and assist individuals throughout the SOAR process.
  9. Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options.
  10. Housing Outreach Liaisons shall attend all Housing Outreach Liaison conference calls, webinars, trainings, and meetings as arranged by the DBH or the Missouri Behavioral Health Council.
  11. Housing Outreach Liaisons shall provide training to internal agency staff and external community stakeholders about housing related best practices.
  12. Develop, monitor and update, individualized housing plans for clients identified as homeless or at-risk of becoming homeless.
  13. Assist Integrated Treatment Team in working with clients in housing crisis to develop a plan and access internal and external housing resources.
  14. Provide prevention services by regularly educating staff and residents on mental health, trauma, co-dependency, stress management and other mental health issues.
  15. Maintain community outreach by networking with community organizations related to recovery housing, public housing, low-income housing, etc.
  16. Participate in regular staff meetings and trainings.
  17. Maintain client files, fill out all pertinent forms, share pertinent information with key staff, keep ongoing case notes of client’s progress and maintain case record in accordance with agency standards.
  18. Work closely with client’s primary counselor and case manager to facilitate client’s transition into services.
  19. Become involved in professional and community organizations when possible.
  20. Provide regular, scheduled “on-call” service, for crisis intervention.
  21. Maintain statistical data for agency reporting.
  22. Adhere to professional standards of practice. Appropriately represent The Salvation Army in the community and be responsible to and be supervised by the Administrator and Clinical Supervisor.
  23. Maintain confidentiality, practice Salvation Army code of ethics and comply with Salvation Army Safe From Harm guidelines.
  24. Maintain positive work atmosphere by acting and communicating in a manner that fosters good relationships with clients, co-workers, and supervisors. This includes, but is not limited to, such actions as: resolution of conflicts in a professional manner; courteous treatment of staff, visitors and clients; respect of others’ property and person; and professional and appropriate communication to and about clients, co-workers and supervisors.

 Minimum Qualifications: 

 

These liaison positions shall be filled by experienced, qualified professionals who have a Bachelor’s in Social Work, Psychology, Sociology or other related field and/or three years of relevant case management experience in housing..  Must have an outgoing personality with good communication skills, ability to work in a team setting and an ability to work with difficult individuals.  Strong leadership style and good organizational skills required.  Knowledge of The Salvation Army and government, state and community resources is helpful.  Must be computer literate and able to utilize computer for documentation of work.  CPR/First Aid certification required.  Must be a team player, a self-starter, able to handle several projects at the same time and perform well with minimum supervision.  Must demonstrate ability to think independently and exercise sound judgment.  Requires a high level of confidentiality.  Must have a valid drivers’ license and pass TSA MVR check. Will be required to obtain CDL.
