Your Specific Responsibilities:

This is professional work providing basic social services to individuals and families in the Healthy Start Program. Incumbent may be required to work during or beyond normal work hours or days in the event of an emergency. Emergency operations functions required of the incumbent include but are not limited to special needs shelters, Red Cross shelters, or other emergency functions, including responses to bioterrorism or threats of bioterrorism. Designated “Need to Know” position with access to the following confidential information: Client medical records. Has key access to Healthy Start records. Has Computer access to HMS system, FL Shots, and Well Family System. Adheres to Florida Department of Health Citrus County Confidentiality and Security policies and procedures. Attends Annual Confidentiality and Security Trainings and updates. Must have a valid Florida driver’s license.

Maintains a caseload and provides Case Management to at risk pregnant women, infants, and their families.

Conducts assessments, develops an Individualized Plan of Care and Family Support Plans.

Provides referrals and assistance in obtaining needed community resources such as social, medical, education, employment, training, daycare services, and housing.

Conducts visits in client homes, clinics, in public, by telephone or letter following an individualized plan of care. Use of personal vehicle is necessary for home visits. Travel is reimbursed at current state mileage rate.

Educates and counsels individuals and /or families about prenatal & infant care, infant development, parenting skills, as well as other prenatal and infant information.

Uses screening tools to conduct Developmental, Depression, Intimate Partner Violence, Substance Abuse, and Tobacco screenings, follow-up providing appropriate Curriculum or referrals as indicated.

Provides services using required curriculum for smoking cessation, breastfeeding, inter-conception care, and parenting as appropriate.

Provides client services in compliance with the Healthy Start Standards & Guidelines and Department of Health protocols.

Based on budget and training availability within one year: Obtain Child Passenger Safety Technician Certification and provide car seat installations and inspections. Obtain Certified Lactation Counselor Certification. Obtain Lamaze Childbirth Educator Certification and to provide Childbirth education services.

Documents all activities accurately and in a timely manner using electronic documentation in the Well Family System as well as provide paper documentation forms including Family Support Plan, Department of Health referral form, Release of Information, correspondence with providers and other agencies. Will maintain accurate records of daily activities in HMS.

Attends Healthy Start staff meetings to ensure compliance with guidelines.

Serves as liaison with community agencies and service organizations to determine the results of services rendered to clients.

Represents Department of Health and Healthy Start at community meetings, board meetings, and health fairs as assigned.

Participates on Boards, Coalitions, and workshops regarding assigned area of responsibility.

Provides education to community organizations to promote Healthy Start awareness and support.

Performs other duties as requested by Supervisor.

Required Knowledge, Skills, and Abilities:

Ability to maintain client records. Ability to understand and apply the methods and protocols used in maternal and child health. Ability to interview clients to identify needs. Ability to provide information and referral services. Knowledge of home visiting. Ability to plan, organize and coordinate work activities. Knowledge of community resources. Knowledge of the principles and techniques of effective verbal and written communication. Ability to actively listen to others. Knowledge of personal computers and software.

Qualifications:

Minimum - Florida Driver's License.

Required – Associates Degree (with 3 years of public health/maternal-child experience) or Bachelor’s Degree in Social Services, health related field, or social work.

Preferred - Bachelor's Degree in Child Development, Education, or Social Sciences, or two years of college and three years of public health/maternal-child experience with case management experience working with young children and families in a human services or early childhood education setting. Certified Lactation Counselor. Lamaze Certified Childbirth Educator.
