The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit. We even have an in-house Therapy Team. This close-knit family of employees is welcoming and always willing to jump in to help out when you need it. What makes us unique:

Job Description
What you will do in this role:
- Officially we title this job an Admissions Coordinator, but in other centers it may be called an Admissions Rep, Intake Coordinator or Admissions Liaison.
When you are a patient, the transition from hospital to SNF is stressful. As an Admissions Coordinator, you will help to organize multidisciplinary services so the patient/resident can safely transition into our center. This could involve coordinating communication with the hospital medical staff, our internal clinical team and balancing insurance needs. You will juggle many things at once: One moment you may be analyzing the cost of meds, the next could be touring a family member, and the next is ensuring your intake packet is correctly signed.
- You will do whatever is needed to help us build our census and meet the needs of the patient.

Qualifications

What we need from you:
- Prior experience as an admissions coordinator/director in a Skilled Nursing Facility is a must.
- Associates degree or higher is preferred
- Experience with Point Click Care (PCC) is ideal
- Working knowledge of federal & state regulations, along with reimbursement & insurance verification experience (Managed Care, Medicare & private insurances)

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
