PT Support Broker

We are looking for an experienced Supports Broker to join our dedicated team. The ideal candidate will assist with the development and implementation of the Support Brokerage Services. The Support Broker serves as the agent of the participant or family to assist in identifying immediate and long-term needs, developing options to meet those needs and accessing identified supports and services.

Essential Job Functions:

· Manage a Workload of 10-15 Program Participants

· Maintain required contact with assigned participants, including telephone calls, email and

quarterly home visits.

· Respond to participant inquiries or concerns in a timely manner.

Provide participants with practical skills training in areas such as:

· Identify community resources, services, and supports to meet their life goals

· Purchasing goods and services

· Managing a monthly budget

· Develop their spending plan and schedule their employees to stay within their monthly budget

Assist participants to:

· Enroll in the Personal Preference Program & complete required enrollment paperwork

· Assist direct care workers, to complete required paperwork to be employed by the participant

· Monitor participant health and safety through monthly phone contact and quarterly home visits

(as needed)

· Maintain progress notes in accordance with NASW & DDD standards

· Report and respond to participant complaints and grievances

· Act as mandated reporter for allegations of abuse, neglect, and exploitation

Minimum Qualifications:

• Must have a BA or BS degree in related field
• Must have at least 2 years of experience in the Developmental Disability field.
• Must have a valid New Jersey driver’s license in good standing
• Must have own transportation and transport participants as needed
• Strong verbal and written communication skills.
• Strong interpersonal relationship skills.
• Desire for continued learning opportunities, and willingness to embrace instruction.
• Ability to work with multi-disciplinary team members, and a desire to work with young adults.
• Reliable attendance record.
• Knowledge of HUD programs a definite plus
• License/Certification: BOGG’s Certification preferred

Woodbridge, NJ: Reliably commute or planning to relocate before starting work (Required)

Work Location: In person/Travel up to 75% in the field
