We are looking for a passionate Clinical Nurse Liaison Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.
POSITION SUMMARY: The Clinical Liaison is responsible for developing and maintaining referral relationships with acute care facilities across the network. Responsible for developing census as defined through targeted goals of the business plan. Serves as the admissions contact for the acute care facilities collecting all pertinent information needed to admit the patient for hospice services.
ESSENTIAL DUTIES:
• Responsible for generating appropriate referrals from a variety of sources by meeting with physicians, discharge planners, long term care facilities, hospitals, etc. to promote hospice services
• Admit GIP patients and provide case management of patients in the hospital and selected skilled nursing facilities.
• Respond to referral sources in a timely manner; evaluate patient appropriateness for hospice; meet with families/patients to discuss goals and explain services of hospice.
• Educate referral sources and follow up on pending patients; maintain community visibility to build relationships with community members and referral sources.
• Communicate and provide feedback to admission staff on referrals.
• Complete assessment of potential hospice patient if needed and document assessment in EMR within required timeframe.
• Partner with Community Liaisons and Clinical Operations (DCS, ED, Team Leads) within the assigned territory to develop interfacing strategies.
• Attend and participate in routine staff meetings and weekly team conferences in person or via media provided as requested.
• Participate in and/or provide in-service training, attend various activities, and other duties as required or assigned.
• Maintains a high level of confidentiality at all times due to the access to sensitive information.
• Maintains regular, consistent attendance and is flexible to meet the needs of the department.
• Follows all Medicare, Medicaid and HIPAA regulations and requirements.
• Always abides by all regulations, policies, procedures and standards.
• Performs other duties as assigned
QUALIFICATIONS:
• Registered RN in the State of Texas
• Nursing experience or medical training required (ability to read charts, know medications, etc.)
• Marketing experience preferred 1-3 years of experience in healthcare / hospice marketing preferred
• Hospice experience strongly preferred, minimum 1 year
• Excellent verbal and written communication skills
• Proficient skills using a computer is required
