Job Summary:
The Registered Nurse (RN) is responsible for coordinating and providing quality monitoring functions and professional supervision of the facility's nursing activities. The RN ensures compliance with all healthcare regulations and best practices, collaborates with an interdisciplinary team, and provides direct and indirect healthcare services to individuals with intellectual and developmental disabilities.
Key Responsibilities:
- Nursing Supervision: Coordinate and oversee nursing activities within the facility, ensuring quality monitoring functions and adherence to best practices. Serve as a key representative on the facility's management team.
- Pre-Placement Assessments: Conduct pre-placement screenings for prospective clients and complete comprehensive nursing assessments prior to admission.
- Healthcare Coordination: Assess and evaluate acute and chronic medical/behavioral conditions and ensure appropriate care interventions are implemented.
- Regulatory Compliance: Ensure adherence to all relevant healthcare policies, laws, and regulations governing IDD services.
- Medication Management: Provide competency-based training to staff on medication administration, conduct routine medication audits, and complete root-cause analyses of medication errors.
- Restricted Health Care Plans: Develop and implement Restricted Health Care Plans in compliance with Title 22 regulations, including training staff and ensuring ongoing compliance.
- Health Monitoring: Continuously assess and monitor client health status, including triaging and arranging for community medical services as needed.
- Staff Training and Development: Educate staff on nursing best practices and medical care, ensuring the team is well-prepared to address clients' health needs.
- Client Psychoeducation: Provide individual and group psychoeducation to clients and families on various health-related topics, including medication management, self-advocacy, and general wellness.
- Interdisciplinary Team Collaboration: Participate in Individual Behavior Support Team (IBST) meetings and serve as the primary nursing representative. Ensure medical concerns are integrated into behavioral treatment plans.
- Clinical Support: Carry out physician's orders and provide clinical support, including drawing blood samples for laboratory tests, coordinating with pharmacies, and overseeing infection control procedures.
- Management Team Participation: Function as an integral member of the management team by contributing to policy development, research, and strategic quality improvement initiatives.
- Direct Client Support: Provide direct nursing services to clients as needed to meet contractual staffing obligations and maintain required certifications and trainings.
- Quality Assurance Initiatives: Actively support Thrive Care Services' commitment to quality by participating in committees, implementing best practices, and promoting continuous quality improvement initiatives.

Qualifications:
Education and Experience:
- Must possess a valid permanent California license as a Registered Nurse (RN) as approved by the California Board of Nursing.
- Preference for applicants with a Bachelor's Degree in Nursing (BSN) and at least two (2) years of experience working with individuals with intellectual and developmental disabilities.
- Certification in Professional Crisis Management (PCM), Ukeru, First Aid, and CPR (including hands-on instruction) required prior to working with clients.
- Must complete fingerprint clearance (Livescan) and a health screening, including a tuberculosis test, and Drug screening, required prior to employment.
- Ability to stand, sit, bend, lift, climb, stoop, kneel, and move intermittently. Must be able to frequently lift/move up to 50 lbs, and occasionally lift more than 50 lbs.
- Valid driver's license and proof of current automobile insurance.
