Job Description:

Seeking a dedicated and experienced Travel Social Worker to support hospital-based social work consults across an inpatient care setting. The role is ideal for professionals who thrive in a fast-paced, interdisciplinary team environment with strong communication and discharge planning skills.

Requirements:

• Minimum 2 years of recent hospital social work experience
• Active compact state license at the time of submission
• BLS certification
• BSW or higher (MSW preferred)
• Proficient in discharge planning, guardianship, surrogacy, and long-term care placement
• Comfortable supporting patients and families during complex care transitions

Unit Overview:

• Bed Coverage: Covers social work consults hospital-wide
• Patient Load: Average caseload of 25–40 patients
• Team Composition: 3 Social Workers, 10 RN Case Managers, 1 Manager, 1 Director
• Systems Used: Expanse & CarePort
• Workdays: Monday – Friday (No weekends)
• Floating: As needed within social work assignments
• Dress Code: Business casual

Preferred Skills:

• Experience in inpatient case management
• Ability to work independently and collaboratively
• Strong documentation and communication abilities
• Knowledge of hospital workflows and care coordination processes
