We are growing and are seeking a LPN to meet the needs of our clients.
This position promotes optimal health and safety of people supported by Tobosa through provision of nursing services. Performs all duties within individual scope of nursing practice and in compliance with the State of New Mexico Nurse Practice Act. Building mutually productive relationships with people supported by Tobosa and co-workers is a key function of this position.
SUPERVISION: Works under the supervision of the Program Director.
GENERAL DUTIES AND RESPONSIBILITIES:
- Treat people supported by Tobosa with dignity and respect, especially in the areas of confidentiality, choice, privacy, positive training methodology, cultural sensitivity, age appropriateness, and ego development.
- Reference, understand, and follow all applicable Tobosa policies related to this position.
- Arrive at work promptly, work flexible schedule, and work as a member of a team in order to assure positive outcomes for participants.
- Complete work on a timely basis assuring deadlines are met and work is completed by the time required.
- Communicate in an open, honest, respectful manner on an ongoing basis with co-workers and supervisors regarding relevant person, program, and employment issues.
- Accept supervisory guidance and direction and use feedback constructively.
- Provide transportation to people supported by Tobosa in company vehicle, when necessary.
- Follow health and safety policy and procedures, especially those related to this position. Remain alert for health and safety concerns/problems and immediately report known problems/infractions for remediation. The safety of people supported by Tobosa is of utmost concern.
- Other duties as assigned by supervisor.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Perform nursing duties in accordance with caseload assignment.
- Maintain the highest level of nursing documentation as guided by best practices for documentation standards by the mainstream healthcare industry.
- Maintain a flexible case load of low to moderate acuity patients.
- Prepare for and attend all ISP, semi-annual, and IDT meetings for the people on the assigned caseload.
- Follow up on medication errors to ensure the completion of the necessary documentation in a timely manner, using Tobosa policies and procedures relating to medication errors.
- Perform quality assurance reviews on medication administration records monthly for people on the assigned caseload.
- Conduct head-to-toe nursing assessments on a weekly basis, or more often as indicated by acuity level, for patients on the assigned caseload.
- Maintain a current and thorough medical history and medication list for each person on assigned caseload.
- Provide Addendum B training for employees for those individuals on the assigned caseload.
- Maintain and document contact with people on the assigned caseload in a variety of settings, including but not limited to, the residential site, day habilitation site, physician and psychiatric appointments, and community events.
- Other nursing duties necessary as indicated by the specific and unique healthcare needs of the individual.
- Perform charge nurse duties on a rotating basis
- Maintain the highest level of nursing documentation as guided by best practices for documentation standards within the mainstream healthcare industry.
- Process physician orders via telephone and/or written orders.
- Triage walk-in and telephone contacts from people supported by Tobosa and employees.
- Provide basic first aid for people supported by Tobosa and employees.
- Dispense and document over-the-counter medications people supported by Tobosa and employees.
- Arrange urgent care medical services for people supported by Tobosa.
- Assist in monthly medication counts, sign-in and delivery of people’s medications.
- Accompany the consulting pharmacist during quarterly inspections of the residential sites.
- Administer medications to children at Los Pasitos, when necessary.
- Other duties as assigned by the Program Director
III. Provide on-call services for the nursing department.
- Serve as the on-call nurse on a regular basis (at least 15 days per month).
- Respond to emergency calls as soon as possible.
- Provide on-site nursing care /support to people supported by Tobosa, as determined by individual crisis plans and emergency medical services protocols.
Minimum Qualifications:
- Current NM licensure as a licensed practical nurse
- Experience caring for people with developmental disabilities preferred
- Must have a current NM driver’s license and provide own transportation during “on call” services
- Bilingual preferred

Salary: $50,000.00 - $70,000.00 per year
