The Client Admissions and Assessment Counselor is responsible for overseeing the entire intake process for new clients, including coordinating admissions, conducting assessments, establishing and maintaining relationships with referring entities, and ensuring compliance with all internal and external protocols. This role acts as a liaison between clients, referring agencies, funders, and internal teams to facilitate seamless onboarding, treatment planning, and record-keeping. Job Responsibilities (not all-inclusive)
- Serve as the primary point of contact for new client admissions, providing information about programs and the admissions process to clients and referring agencies.
- Establish and maintain professional relationships with referral sources and third-party referring entities.
- Coordinate the scheduling and conduct of intake interviews and assessments, utilizing skills in screening, intake, orientation, and treatment planning.
- Collect and review client documentation necessary for intake screening and ensure proper submission to the review team for approval.
- Facilitate the creation and maintenance of client records in compliance with Minnesota Statute 245G, including creating and updating files within the Electronic Medical Record (Procentive).
- Determine client financial responsibility and monitor changes with funders, submitting required notifications within specified timeframes.
- Schedule and coordinate new client admissions, including informing clients and relevant staff and attending to follow-up activities.
- Monitor and manage wait lists, census logs, and intake progression spreadsheets to ensure operational efficiency.
- Conduct comprehensive assessments on potential and incoming clients, including screening, intake, orientation, and treatment planning, adhering to the Minnesota Matrix Six Dimensions and ASAM Criteria.
- Participate in crisis intervention, client education, case management, or individual counseling as needed.
- Collaborate with clinical staff and external professionals regarding client treatment, discharge procedures (note: clients cannot be discharged at staff request unless explicitly instructed by the Treatment Director or Clinical Supervisor), and service delivery.
- Complete all required paperwork related to admissions, orientation, and assessments promptly and accurately.
- Monitor daily census logs and follow internal protocols for client care, documentation, and confidentiality.
- Attend staff in-services, meetings, and other related trainings to stay up-to-date on policies, procedures, and professional development.
- Assist in reviewing admissions and ensure all documentation and assessments are complete and compliant before proceeding.
- Perform other duties as assigned to support the overall mission of the organization.
Requirements:
- Must have a Minnesota License for Alcohol and Drug Counseling or Temporary Permit/License through the Board of Behavioral Health and Therapy.
- Must know and understand implications of Minnesota Statutes chapter 245G and Minnesota Statutes; chapter 245A (Human Services Licensing Act-HSLA), sections 626.556 (Maltreatment of Minors Act-MOMA), 626.557 (Vulnerable Adult Act-VAA), and 626.5572 (VAA definitions).
- Must be willing to submit to and pass DHS Background Study.
- Must have basic computer skills and ability to use office equipment such as copier, scanner, and printer.
- Must provide 12 hours of co-occurring education/training within 6 months of hire.
- Must obtain 8 hours of co-occurring education/training on an annual basis
