Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
The schedule for this position is Monday-Friday 8:30am-5:00pm.

A Successful Social Worker has the following:
- Provide psychosocial assessments of patients and families to identify emotional, social, cultural, financial and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
- Support patient/family’s individual spiritual/cultural beliefs by assessing for cultural issues; developing culturally sensitive care plan and collaborating with interdisciplinary team in provision of culturally sensitive care.
- Formulate, develop, and implement a comprehensive psychosocial plan of care utilizing appropriate clinical social work treatments and intervention, while maintaining the dignity of the patient.
- Responsible for documenting assessments, care plans, interactions, and interventions according to regulatory standards and agency standards for service lines associated with the identified patient.
- Maintain awareness of patient payer source and assist the finance department in collecting accurate insurance information, including facilitating self-pay patients through the appropriate financial screening process (i.e. Medicaid, Charity Care,).
Responsibilities:
- Provide high-quality and compassionate care to patients per their individualized care plan
- Works collaboratively with the interdisciplinary team (IDG) in achieving patient care goals
- Maintain comfort, privacy, and dignity of our patients
- Promptly reporting pertinent information to the immediate supervisor
Requirements:
- Master of Social Work from an accredited university required
- 2 years-experience in hospice or working with end of life preferred
- LCSW preferred
- Must have reliable transportation
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
