Job Summary
The Community Outreach Coordinator is primarily responsible for sourcing, establishing, and maintaining relationships with client referral sources and prospective clients. The community Liaison is the face of Treasure Health in the local market. The position is responsible for providing outreach and education regarding the Agency's services to potential clients and referral sources and bringing new clients into the Agency.
Responsibilities and Duties
Develop new referral sources by planning and organizing a schedule to call on potential referral outlets and other sources of prospective clients.
Meets with prospective clients in their homes to discuss the agency's services, payment options, and onboarding process.
Assists the agency s Client Relationship Managers in processing and on-boarding new clients.
Communicates with and supports existing clients as needed with our onsite Client Relationship Managers.
Keeps management informed by updating our tracking systems and submitting referral activity and results reports, such as daily call reports, weekly work plans, and monthly and annual territory analyses.
Develop out-of-the-box community outreach strategies/models and evaluate their effectiveness.
Assist with developing recruitment strategies to attract candidates.
Collaborates closely with Recruiting and Human Resources to ensure candidates complete the onboarding process.
Other duties as assigned.
Requirements:
Understanding of Managed Care Organizations.
Proven relationships with assisted living facilities, senior centres, etc.
Bilingual candidates are encouraged to apply.
Customer service focused.
Schedule:
- Monday to Friday
- Occasional evening and weekend work may be required as job duties demand.
