White Light Behavioral Health, a part of National Behavioral Health provider TreatmentX is opening a new location in Columbus, OH. White Light provides a full continuum of care treatment model for adults with substance use disorder in Ohio.
We are hiring Intake and Discharge Planners for day shift and swing shift to include 4 day, 10 hour schedules.

POSITION PURPOSE:
Assures that services are provided in support of the ongoing provision of quality care and client movement to the least restrictive, least intrusive service setting. Responsible for the orientation of new patients to the facility and assuring all consent forms, authorizations, identification, and screenings are completed that are required for patient admission. The intake/discharge specialist is responsible for the inventory of patient valuables and assuring safe check in and return of valuables. The intake specialist may also be responsible for transportation of a patient to or from the facility at the time of intake or discharge.
ESSENTIAL JOB FUNCTIONS:
- Accountable for leading a WE CARE culture that focuses on our mission to provide a transformational influence to patients and their loved ones.
- Organizes and facilitates Treatment Team Meetings twice per week. Maintains and distributes meeting minutes.
- Develops and facilitates the implementation of short-term crisis stabilization plans. Assures that clients and, as appropriate, family/others are involved in Individualized Service Plan (ISP) development and implementation.
- Reviews service plan and performs follow-up to determine quantity and quality of service provided.
- Completes individualized weekly progress summaries.
- Accesses and records client and community resource information. Compiles records and prepares reports as required or needed.
- Facilitates a smooth transition into treatment and works to ease patient anxiety during the admissions process.
- Shares pertinent information to teams as obtained from clients and families.
- Monitors appropriate communication of information between Clinical Team, Nursing Team, Medical Team and Admissions Department.
- Provides individual and group community support and crisis intervention services for patients as needed.
- Coordinates provision of mental health (MH) and substance abuse (SA) diagnostic assessment,nursing assessment, and other pre-admission screening.
- Facilitates required linkages and referral of clients to community resources and other organizations.
- Conducts new patient orientation and admissions.
- Instructs patients in completing new admissions packet.
- Assists in medical record maintenance and ensures set-up of new admissions supplies.
- Provides copies of medical records to approved parties with prior approval of compliance director.
- Maintains patient confidentiality laws (HIPAA).
- Transports patients to and from appointments and meetings when necessary.
- Works as scheduled by the Executive Director. Provides BHT and administrative back-up including residential services and emergency coverage as needed.
- Contributes to the building of a positive team spirit and identity which will enable staff to interact effectively with both their own support and professional staff as well as with other support and professional staff.
- Provides and/or participates in clinical training and staff development opportunities.
- Submits timecards, service documentation, required reports, and activity logs in accordance with policies, procedures and protocols.
- Participates in supervision process, staff meetings and case reviews.
- Other duties may be assigned.
POSITION REQUIREMENTS & QUALIFICATIONS:
EDUCATION: High school diploma required. Bachelors degree in counseling, social work or other preferred.
EXPERIENCE: At least 3 years of experience working with adults and leadership in group and individual service settings is required.
LICENSURE/CERTIFICATION: Annual CPR review and PPD testing, valid Ohio driver’s license and use of personal car.
KNOWLEDGE, TRAINING, FUNCTIONAL COMPETENCIES REQUIRED AT TIME OF HIRE:
LANGUAGE SKILLS: Ability to explain diagnostic and treatment information in an appropriate fashion to colleagues, physicians, and patients. Ability to respond to common inquiries or complaints from patients. Ability to write clear and concise documentation that conforms to prescribed style and format. Ability to effectively present information to management.
MATHEMATICAL SKILLS: Basic addition and subtraction. Ability to count and record financial transactions.
REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions
