Salary: $46000 / year

POSITION OVERVIEW:
The primary responsibilities of the Program Specialist include providing intensive and specialized supports for people who have an intellectual disability or mental illness. Program Specialists focus on domains such as personal care, recreation/leisure, vocational, safety, social, and/or community awareness and integration. Program Specialists provide individual supports to persons who have extraordinary behavioral or medical support needs.

The Program Specialist works directly in multiple residential or community day services locations and is assigned to a Trinity network. The working hours for this position are 1:00pm to 9:00pm, 3:00pm to 11:00pm or 4:00pm to 12:00am for the residential sites and 8:00-4:30pm for the community day services. The specific shift is determined by when the department's afternoon shift starts or when the community day services start.

ESSENTIAL FUNCTIONS:

Participates and interacts with people supported in various activities as specified in the Implementation Strategy Plans, Skill Acquisition Plans, and Social Transition Plans.
Follows through and implements all required programs, outcomes, goals, treatment plans, skill acquisition plans, social transition plans, and outings.
Implements Safety Care de-escalation and crisis management strategies as trained.
Collects and records all required data.
Actively provides individualized engagement with people supported throughout each shift.
Enhances the lives of the people supported, creates and facilitates opportunities for people to flourish, increases autonomy, independence, and choice making, facilitates progress towards achieving goals and outcomes.
Develops and maintains positive relationships with people supported, supervisor, leadership staff, and Independent Living Coaches to allow for successful collaboration.
Maintains a warm, positive demeanor in helping individuals with developmental disabilities.
Shows reliable attendance.
Completes all required documents in a timely manner (e.g., Medisked completed prior to end of shift, clinical referrals immediately following appointments, mileage by the 5th, receipts within 3 days, injury/incident reports within 24 hours, etc.); paperwork and electronic documents are completed accurately.
Consults with Behavior Analysts, medical professionals, and Qualified Intellectual Disability Professionals concerning appropriate goals, objectives, and treatment techniques.
Completes or follows medication administration procedures and specialized medical protocols across multiple locations.
Follows agency protocols for persons with specialized medical care.
Assists with development and implementation of age appropriate and functional training materials.
Makes suggestions for improving organization and program effectiveness and efficiency.
  • Transports persons served to and from appointments and activities as needed.

SKILLS AND ABILITIES:

  • Able to make independent decisions and problem solve
  • Able to deal tactfully with colleagues, program participants, family members, visitors, government agencies/personnel, and the general public
  • Able to implement goals, objectives, policies and procedures, necessary for providing quality care
  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Strongly self-motivated

PHYSICAL DEMANDS:

  • Be able to lift over 40 pounds from floor to waist unassisted
  • Be able to push and pull 75 pounds
  • Be able to complete deep knee squats
  • Be able to push wheelchairs up to 200 pounds
  • Remain in a stationary position, often standing or sitting for prolonged periods
  • Operate motor vehicles
  • While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach, stoop kneel to install computer equipment
  • Physical activities that could be used include: driving, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, and repetitive motion.

QUALIFICATIONS:

  • Bachelor's degree in a human service field or similar
  • Valid Driver's License
  • Pace Certification
