Position Summary:

The Community Health Outreach Worker is responsible for providing outreach and education services focused on health awareness to improve health literacy, healthcare access for underserved communities and to increase knowledge among the community. The Community Health Outreach Worker also works collaboratively with TrueCare departments as well as community-based agencies to make critical linkages to TrueCare health information and services for potential clients and patients.

Qualifications:

• Associate’s Degree in healthcare or related field or equivalent work experience.
• At least one (1) year experience in health care or related field performing outreach, education, case work, and/or customer service to diverse populations.
• Knowledge of local community-based agencies.
• Availability to work regularly outside of normal shifts (i.e., 8am to 5pm).
• Public speaking, computer literacy, including internet navigation and research skills, and proficiency with the Microsoft Office suite, including Outlook, Word, Excel and PowerPoint.
• Clean driving record, proof of insurance and CA drivers license - no tickets/accidents- will be driving to venues, need reliable transportation.

QUALIFICATIONS DESIRED

• Bachelor’s Degree in healthcare or related field.
• Bilingual in English and Spanish.
• Local candidates will be considered

LICENSES

Will be required to travel to all NCHS’ sites and community locations therefore California Driver’s License, proof of insurance and clean driving record required.

Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical to allow for future and continued salary growth.
