Patient Types: Terminal (hospice patients) come in for either respite, GIP (general inpatient) or routine level of care
Orientation: (facility and unit) Hospice house, 224 Elk Street
Scheduling: 8 or 12 hour shifts, needs to work varied shifts, up to 40 hours per week
Weekend rotation - every other weekend
Will they be on call? If so, what is that schedule? No call
Holiday Expectations: yes
Approval of time off? needs approval through contract
7 days maximum time off
2 references from last 12 months (Manager/Supervisor reference type) - required for submission

Experience
- 12 months of role experience is required with some in the last 12 months.
- 18 months of Hospice experience is required with some in the last 24 months.
- Experience with Epic is preferred from any number of months.
- Candidates must have a license (required for submission).
- Travel only, local not allowed. Candidates must live >100 miles from facility to be submitted.
- COVID vaccination required after submission. Religious and medical declinations accepted.
