Position Overview: We are seeking a dedicated and experienced RN to provide skilled nursing care to our homebound patients. The Home Health RN will assess patients’ needs, develop individualized care plans, and coordinate care with interdisciplinary team members to optimize patient outcomes. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for delivering high-quality, patient-centered care in the home setting.
Key Responsibilities:
- Conduct comprehensive assessments of patients’ health status and develop individualized care plans based on assessment findings, physician orders, and patient goals.
- Provide skilled nursing care, including medication administration, wound care, catheter care, IV therapy, and other nursing interventions as prescribed by the physician.
- Monitor patient progress, assess for changes in condition, and communicate with physicians and other healthcare providers to ensure timely and appropriate interventions.
- Educate patients and families about their health conditions, medications, and self-care techniques to promote independence and prevent complications.
- Document all patient care activities accurately and in a timely manner, adhering to regulatory requirements and organizational policies.
- Collaborate with interdisciplinary team members, including therapists, social workers, and home health aides, to coordinate care and maximize patient outcomes.
- Provide emotional support and encouragement to patients and families, fostering a caring and supportive environment.
Qualifications:
- Registered Nurse (RN) license in the state of Texas.
- Minimum of 1 year of nursing experience, preferably in home health or a related field.
- Strong clinical skills and knowledge of nursing principles and practices.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and team members.
