General Purpose of the Job

Responsible for all job coaching service delivery to individuals with disabilities in all employment models (individual, enclaves, affirmative business).    18 Hours per week - flexible schedule.

Essential Duties and Responsibilities

1. Intensive and follow-along services, including but not limited to: specific job skills training, work behaviors, enhancing natural supports, work benefits, instruct in all areas needed to maintain employment including but not limited to grooming, travel, benefits and entitlements, job restructuring, job accommodations, budgeting, living arrangements, social/recreational, self-advocacy.
2. Communicates to employers and co-workers to facilitate the development of natural supports. 
3. Maintains all agency Policy & Procedures, including Ethics and Confidentiality for employment implementation
4. Responsible for timely completion of all service documentation as required by OMIG and agency procedures. Documentation is completed via paper charts and computer database entry.  
5. Works closely with clients, families, referring agencies, service coordinators, other community employment and vocational rehabilitation staff under supervision.
6. Attends all required meetings and training.
7. Must operate own vehicle between job sites and agency sites.  May include driving agency van to transport persons served to and from interviews assessment sites and/or job sites, this is an essential function of position.
8. Any other related responsibilities as assigned by your supervisor.

Computer Skills                                                

To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Word, Outlook, Excel).

Qualifications

Certificates, Licenses, Registrations

Current First Aid Certification or willingness to obtain within 3 months from the date of hire.  It is the employee's continuing responsibility to maintain certification at their own expense.  Valid clean NYS Driver's License

Education and/or Experience                                                   

High school diploma or general education degree (GED); or one to three months' related experience and/or training; or equivalent combination of education and experience.

Salary : $17 - $19
