Are you an experienced direct care provider looking for rewarding work with children or adults with disabilities in the community? As a Supported Living Care Navigator you will be responsible for providing case management for Supported Living members including oversight of the PCA program and community integration and supervision of assigned staff. Some afternoon and evening hours may be required.

Qualifications:
- Bachelor’s degree, preferably in a field of human services
- Two or more years experience including PCA services and case management services
- Familiarity with community resources and experience facilitating programs for the public preferred
- Training and supervisory experience preferred
- Working knowledge of age appropriate activities and behaviors and treatment modalities and assessments
- Basic computer skills, working knowledge of remote communication tools like Zoom preferred
- Technical writing
- Defining issues and drawing conclusions
- Planning and problem solving
- Appropriate communication skills to effectively communicate with members, parents and professionals
- Understanding of the importance of supporting the dignity and human rights of all members
- Ability to travel to conduct components of essential job duties.
- Valid driver’s license, availability of private vehicle, up-to-date car insurance and proof of annual vehicle inspection required.
- Ability to work flexible hours in order to accommodate the needs of members
- Satisfactory CORI, SORI, DPPC and National Fingerprint Check
- A negative Mantoux TB test or a clear chest x-ray
